经皮穿刺氩氦超导靶向治疗头面部肿瘤

摘　　要：目的观察氩氦超导靶向治疗头面部肿瘤的可行性和安全性。方法在B超或CT引导下,依据影像学资料所提示的进针方向、进针角度、进针深度,将氩氦刀插入瘤体内,启动氩氦超低温手术系统,进行治疗。结果5例头面部肿瘤(包括腮腺瘤2例,颌下腺癌、上颌窦癌和眼睛黑色素瘤各1例)接受氩氦超导靶向治疗,肿瘤冷冻面积均在90%以上。术中较安全,无一例围手术期死亡。头面部肿瘤表浅部位有2周左右的红肿期。3例患者发生面神经麻痹(面神经附近冷冻),持续1个月后渐缓解。3个月后肿瘤均有不同程度的缩小。结论氩氦超导靶向治疗是一种安全、有效的治疗头面部肿瘤方法,特别适于侵犯大血管或术后复发的患者。Objective To investigate the feasibility and safety of the percutaneous helium-argon superconductance targeting cryotherapy guided by computer tomography(CT) or B ultrasound in treatment of tumors of the head and face.Methods Five cancer patients with parotid adenoma,submaxillary adenocarcinoma,maxillary carcinoma of sinus and eye melanoma received helium-argon superconductance targeting therapy between Augest 2001 and November 2002.The cryoprobe was inserted in the tumor percutaneously guided by CT or B ultrasound with local analgesia without general sedation.The percentage of tumor covered by the ice ball was measured immediately after cryoablation,and the complications were observed.Results The percentage of tumor covered by ice ball was more than 90% in 5 cases.There was no death during cryotherapy,and no severe complications after cryosurgery.Three cases of facial paralysis happened immediately after cryosurgery,and lasted 1 month.The tumors shrank to various degrees 3 months after cryotherapy.Conclusions Percutaneous cryoablation was a minimally invasive treatment of tumors in the head and face.
