摘  要:正确理解角膜映光检查法光学原理,对于正确理解与正确使用角膜映光检查法及以角膜映光检查法为基础的他觉斜角检查方法具有重要意义。我们从几何光学与心理物理学角度论述角膜映光检查法光学原理,总结与角膜映光检查法有关的斜角检查方法,介绍3种特大斜角临床检测方法。论证巩膜映光点来源,求得巩膜映光点每毫米位移约对应4.8°斜角,理论Hirschbergratio约为7.349°/mm;近来美欧等发达国家采用照相或摄像检查方法所得照相或摄像Hirschbergconversionratio约为12~13°/mm(21~22PD/mm)的原因,是因为其所依据的检测参照点—“瞳孔入口中心”在角膜表面的投影位置是随“角膜映光检测点”的移动而移动的,及将其三维空间距离二维平面化测量之故。· It is of great importance to comprehend accurately the optic principle of the corneal light reflection test (CLRT) for correct usage of CLRT and other examination procedure from objective deviating angle that based on CLRT. We discussed the optic principle of CLRT from the perspective of geometrical optics and psychophysics and summarize the examination procedure from deviating angle that is relevant to CLRT. Three examination procedures for outsize deviating angle were introduced in this paper. We also discussed the source of sclera light reflection, and proved that the value is 4.8° per millimeter of sclera reflex displacement, and that the theoretical Hirschberg ratio is 7.349°/mm. In recent years, Hirschberg conversion ratio of 12-13°/mm (21-22PD/mm) was obtained by photographic measurements of corneal reflex positions in developed countries such as USA and Europe. One reason was that the projection point of the pupil entrance center on the surface of cornea followed the movement of corneal reflex point. The other reason was that the measurement of corneal reflex displacement was a two-dimensional plane instead of a three-dimensional space.·
