胃内分泌细胞增生的分类与内分泌肿瘤的诊断

摘　　要：应用１０种内分泌标记或抗体对１６１例胃癌及癌旁粘膜，３４例胃内分泌肿瘤及瘤旁膜粘膜内分泌细胞进行观察。提出内分泌细胞增生（分为轻、中、重度）异型增生（分为轻、中、重度）的诊断标准。并对含有内分泌细胞分化的胃肿瘤的命名进行讨论。这类肿瘤可分为典型类癌；内分泌细胞癌（肿瘤中含有内分泌细胞数量为５０％以上）；胃癌伴有内分泌分化（肿瘤中含有内分泌细胞数量为５０％以下）。胃内分泌肿瘤可应用免疫组化确定分泌激素的类型。Immunohistochemical methods including 10 endocrine markers or antibodies were applied for the investigation of endocrine cells in specimens of 161 cases of gastric adenocarcinoma with its surrounding mucous membrane and of 34 cases of gastric endocrine tumor with its surrounding mucous membrane.The criteria of diagnosis for endocrine cell proliferation (mild, moderate and severe) and dysplasia (mild, moderate and severe) were proposed and the nomenclature of gastric tumors containing endocrine cell differentiation was discussed.The gastric endocrine tumors were classified into typical carcinoid. endocrine carcinoma(containing endocrine cells over 50%)and carcinoma with endocrine differcntiation (containing endocrine cells below 50%). The type of hormone secreted by the tumor can be identified by an immiunohistochemical method.
