摘 要:对4例正常人肝、5例肝硬变、5例肝腺瘤样增生(AH)、16例肝细胞癌(HCC)(Edmondson分级I、II、III级各4、6、6例)组织中突变型p53蛋白(mP53)和甲胎蛋白(AFP)进行了免疫组化检测。结果发现:正常人肝mP53、AFP均阴性。5例肝硬变组织中,4例同时显示mP53和AFP阳性,1例显示二者皆阴性。5例AH中,4例为mP53、AFP阳性,1例为二者阴性。在16例HCC中,4例1级者,2例显示mP53、AFP均阳性,2例为二者均阴性;6例II级者,4例mP53和AFP均阳性,2例二者均阴性;6例III级者,1例mP53阳性AFP阴性,2例mP53阴性AFP阳性,3例二者均阴性。结果提示:p53的突变可能发生在肝细胞癌变过程的早期,与癌变过程的启动有关;其作用可能与AFP的重新激活有关。o investigate the expression of mutant p53 pro-tein (mP53) and alpha-fetoprotein (AFP) duringhepatocarcinogenesis, we detected immunohistoch-emically the specimens from 4 cases of normal hu-man liver. 5 of cirrhosis, 5 of adenomatoushyperplasia (AH), and 16 of hepatocellularcareinoma (HCC) (by Edmondson classification. The4 cases with normal liver showed negative mP53 andAFP. Four of the 5 cirrhosis cases were positive formP53 and AFP. In 5 AH cases, 4 were positive formP53 and AFP. In the 4 cases of grade I HCC, 2were positive for mP53 and AFP. In the 6 cases ofgrade II HCC, 4 were positive for mP53 and AFP. Inthe 6 cases of grade III HCC, 1 showed mP53positive staining but negative AFP, and 2 were nega-tive for mP53 but positive for AFP, while 3 were neg-ative for both mP53 and AFP. The results indicatedthat the mutation of p53 gene occurred in the earlystage of hepatocarcinogenesis, and may be correlatedwith the initiation of hepatocarcinogenesis, and thatmutant p53 protein probably related to thereactivation of AFP gene.
