摘 要:目的通过MRI动态增强扫描,观察垂体泌乳素微腺瘤的特征,与血清激素含量对照研究,分析MRI在肿瘤生物学行为评价中的作用。方法搜集确诊女性垂体泌乳素微腺瘤20例,按血清泌乳素水平分组,血清泌乳素大于等于200ng/mL为高泌乳素血症组,血清激素含量小于200ng/mL为低泌乳素血症组。同时行垂体MRI常规扫描及灌注扫描,选105s时的MRI成像为研究对象,观察肿瘤在垂体内的形态及位置。在瘤体内选取感兴趣区,测量动态增强扫描前后瘤体增强的差值,分析不同血清激素水平肿瘤的血流灌注。结果垂体泌乳素微腺瘤好发在垂体前叶后部两侧,相同时相激素水平较高组血流灌注明显增高,与血清激素较低组相比,差异具有显著性(P<0.05)。结论MR动态增强扫描可准确显示泌乳素微腺瘤的形态及位置,与血清激素含量对照研究可初步评估肿瘤的生物学特性,为临床治疗提供重要的影像学依据。Objective To study the correlation of Gd- DTPA- enhanc ed dynamic MRI and the value of hormone in blood serum in prolactin- secreting microadenoma of the pituitary and to analyse the value of MRI in evaluating tumo r biological character.Methods Twenty patients with pituitary prolactin- secret ing microadenoma underwent noncontrast- enhanced T1- weighted and Gd- DTPA- enhanced dynamic MRI . All cases were divided into two groups according to level of hormone in blood serum (high prolactin valuses group ≥ 200 ng/mL,low prolac tin valuses group< 200 ng/mL) .Results The prolactin- secreting microadenoma o f 17 cases were located in the lateral acidophilic wings of the pituitary.The di spersion of signal intensity between noncontrast- enhanced T1- weighted and Gd - DTPA- enhanced dynamic MRI in prolactin- secreting microadenoma were diffe rent in two groups, the difference was statistically significant (P< 0 .05) .Conclusion The shape and locality of prolactin- secreting microadenoma can be showed in Gd- DTPA- enhanced dynamic MRI clearly .That Gd- DTPA- e nhanced dynamic MRI combined with the value of hormone in blood serum can evalua te the tumor biological character further.
