摘　　要：目的研究血管内皮生长因子(VEGF)与肾母细胞瘤(Wilms′瘤)不良预后的关系。方法用免疫组织化学ELISA法对35例Wilms′瘤患儿术前、术后1周、2周VEGF蛋白的表达水平进行检测,并在4、8、16、32周随访患儿。结果35例Wilms′瘤患儿术前血清VEGF为(89.47±22.45)μg/L。术后1、2周检测血清VEGF为(2.75±0.31)μg/L、(1.52±0.18)μg/L,与术前比较差异有显著性(t=4.125P<0.01)。在4、8、16、32周随访有9例患儿肺转移,其中5例死亡。检测转移与未转移患儿血清VEGF分别为(144.76±42.62)μg/L,(1.25±0.17)μg/L,差异有显著性(t=4.623P<0.01)。结论VEGF血清表达在Wilms′瘤患儿术后显著下降,而在转移时明显升高,说明血清VEGF可作为监测Wilms′瘤预后临床指标。Objective To study the connection between vascular endothelial growth factor(VEGF) and pool prognosis in nephroblastoma.Methods Serum VEGF was measured with immuneohistochemical(ELISA) methods in 35 children of nephroblastoma at preope-ration,1 week,2 weeks after surgery respectively. Simple visiting happened after surgery 4,8,16,32 weeks.The countrol group was 35 healthy children. Result Before surgery,the median VEGF in the children was (89.47±22.45) μg/L.After surgery 1 week,2 weeks,levels in the children fell significantly to (2.75±0.31) μg/L,(1.52±0.18) μg/L.There was a significant difference compared with preoperation ( t =4.125 P <0.01). Nine children had metastasis in lungs through simple visiting after surgery 4,8,16,32 weeks.Five children died among them. Serum VEGF was measured from the deteriorate children and un-deteriorate children:(144.76±42.62) μg/L,(1.25±0.17) μg/L( t =4.623 P <0.01).Conclusion Serum VEGF expression indicate a rapid reduction following surgery,but the level rapidly increase during the metastasis.The findings reveal nephroblastoma′s occurring,developing,and metastasis significently correlated with production of VEGF.VEGF can be used as a clinical marker to predict the adverse prognosis of nephroblastoma.
