食管癌Survivin和PCNA的表达及其临床意义

摘　　要：目的研究Survivin蛋白和PCNA在食管癌中的表达,探讨它们之间的关系,了解其临床意义。方法用免疫组织化学SP法检98例食管癌组织及癌旁正常黏膜的Survivin蛋白和PCNA的表达水平。结果食管癌组织Survivin阳性表达率为80.6%,显著高于癌旁正常黏膜7.1%(P<0.01);低、高分化癌的阳性率有高度显著性差异(P<0.01);淋巴结转移组阳性率为94.1%,高于无淋巴结转移组的阳性率73.4%(P<0.05);小于3年生存期的Survivin阳性率比大于3年生存期的高。癌组织PCNA标记指数LI(72.6±10.2)%;显著高于癌旁正常黏膜的(37.5±11.5)%(P<0.01);有淋巴结转移的LI也比无淋巴结转移的高;Survivin的阳性率越高,PCNA的LI也越高。结论Survivin在食管癌中高表达,癌组织分化越差,阳性率越高,越易转移,预后越差,Survivin与PCNA正相关。Objective To investigate the expression and clinical significance of survivin and proliferating cell nuclear antigen(PCNA) in human esophageal carcinoma. [WTHZ]Methods The expression of survivin and PCNA was detected by immunohistochemical SP method in 98 cases of esophageal carcinoma and paired normal mucosa adjancent to tumor. Results The positive rate of survivin expression in esophageal carcinoma and normal mucosa adjancent to tumor was 80.6% and 7.1%, respectively, which was obviously higher in the former than in the latter(P<0.01). There was a significant difference in the positive rates of survivin expression between the low and high differentiated esophageal carcinomas(P<0.01). The positive rate of survivin expression in esophageal carcinoma with lymph node metastasis was significantly higher than that without lymph node metastasis(94.1% vs 73.4%,P<0.05), and in esophageal carcinoma patients with less than 3-year survival period was higher than in ones with more than 3-year survival period as well. Labeling idex(LI) of PCNA in esophageal carcinoma was obviously higher than that in normal mucosa adjancent to tumor [(72.6±10.2)% vs( 27.5±11.5)%, P<0.01], and was also higher in esophageal carcinoma with lymph node metastasis that without one. The survivin expression was positively related with LI of PCNA in esophageal carcinoma. Conclusion Survivin expression in esophageal carcinomas was high, and was related to the differentiated grade, metastasis and prognosis of esophageal carcinoma.Survivin expression in esophageal carcinoma was positive relation with LI of PCNA.
