摘  要:目的研究内源性生物素在不同组织类型的肾良、恶性肿瘤中的表达及其与预后的关系。方法采用组织芯片技术,结合微波加热修复,用免疫组化SP染色方法进行内源性生物素检测。结果内源性生物素阳性染色定位于细胞质。70例肾肿瘤中,生物素阳性37例(52.9%),阴性33例(47.1%)。9例正常肾组织均为强阳性(100%)。共13例获得随访。肾恶性肿瘤7例,其中4例健在无复发,生物素阳性和阴性各2例;另外2例复发,1例死亡,这3例生物素均阴性。良性肿瘤6例,全部健在,生物素均阳性。结论肾组织含有内源性生物素。不同性质的肾组织含内源性生物素的情况不同:内源性生物素与组织学类型有关,其检出率和强度在肾嫌色细胞癌和肾乳头状肾细胞癌较高,在肾透明细胞癌、肾盂乳头状移行细胞癌、肾母细胞瘤中不存在或很低。提示检测内源性生物素在肾肿瘤的鉴别诊断中可能有一定参考价值。Objective To clarify whether any difference exists in expression of endogenous biotin (EB) in different histological type of renal tumors. Methods Formalin-fixed and paraffin-embedded tissues from 55 cases of malignant renal tumors, 15 cases of benign renal tumors and 9 cases of normal kidneys were studied for endogenous biotin using tissue array combining with microwave heating treatment and streptavidin immunohistochemistry. Results EB was detected in cytoplasm of renal epithelium of normal and tumor tissues. 37 of 70 cases (52.9%) of renal tumors showed biotin positive. All 9 cases of normal renal tissue were strongly positive for biotin. In 55 cases of renal malignant tumor, 24 cases were positive for biotin, including 16/27 (59.3%) cases of papillary renal cell carcinoma, 1/14 (7.1%) cases of renal clear cell carcinoma, 7/10 (70%) cases of chromophobe renal cell carcinoma, 0/2 cases of renal pelvis transitional cell carcinoma and 0/2 cases of nephroblastoma. In 15 cases of renal benign tumors, 13 cases were positive for biotin, including 12/13 (92.3%) cases of oncocytoma and 1/2 (50%) cases of renal papillary adenoma. Conclusions (1) This study has confirmed that renal tissue contains abundant endogenous biotin; (2) The frequency of EB is found to be different in different kinds of renal normal and tumor tissues. The highest frequency of EB is seen in normal renal tissues, and then in renal benign tumors, and the lowest in renal malignant tumors; (3) In renal malignant tumors, frequency of EB is associated with subtypes of tumors. For example, EB is highly detected in chromophobe cell carcinoma and papillary renal cell carcinoma; on the contrary, EB is detected low or absent in renal clear cell carcinoma, renal pelvis transitional cell carcinoma and nephroblastoma. These suggest that detection of EB may be helpful for differential diagnosis of renal tumors.
