子宫内膜癌中癌基因C-erbB-2雌激素调节蛋白雌孕激素受体的表达相关性及临床意义

摘　　要：目的:研究子宫内膜癌中癌基因C-erbB-2、ps2及ER、PR的表达相关性及临床意义。方法:采用原位杂交和免疫组化结合的方法检测了30例正常子宫内膜、30例子宫内膜非典型增生及76例内膜癌组织中C-erbB-2、ps2及ER、PR的表达。结果:C-erbB-2基因在子宫内膜非典型增生中即出现了过度表达,mRNA和蛋白的表达率为56.7%(60%),子宫内膜癌C-erbB-2mRNA和蛋白的表达率增加为76.3%,且C-erbB-2表达与子宫内膜癌的病理分级及病理类型有关(P<0.01,P<0.05)。ps2仅在非典型增生及癌中表达,非典型增生中表达率为90%,内膜癌中为40.8%,ps2表达与病理类型、分级有关(P<0.01,P<0.05)。ER、PR在正常内膜中高表达,非典型增生及内膜癌中表达率下降,且与分级及病理类型相关(P<0.01,P<0.05)。C-erbB-2表达与ps2及ER负相关,ps2与ER正相关(P<0.05)。结论:C-erbB-2基因、ps2在子宫内膜癌发生发展中具有重要作用,C-erbB-2、ps2及ER、PR异常表达及相互间的调节关系对子宫内膜癌的形成及发展有重要意义,可考虑作为预后判断的指标。Objective:To study the expression of C-erbB-2,ps2,ER and PR in endometrial carcinoma and its clinical significance.Methods:The expression of C-erbB-2,ps2,ER and PR was determined in 30 normal endometrium,30 cases of atypical hyperplasia of endometrium and 76 cases of endometrial carcinoma using in situ hybridization and immunohistochemical methods.Results:In atypical hyperplasia of endometrium and endometrial carcinoma,the overexpression rate of C-erbB was respectively 56.7% and 76.3%,which of ps2 was respectively 90% and 40.8%.The expression of ER and PR was higher in normal endometrium than that in atypical hyperplasia of endometrium and endometrial carcinoma.The expression of C-erbB-2,ps2, ER and PR was associated with pathologic grade and type (all P<0.01,P<0.05).The expression of C-erbB-2 was inversely correlated with that of ps2 and ER,while ps2 was positively correlated with ER (P<0.05).Conclusion:The abnormal expression of C-erbB-2,ps2,ER and PR have important role in the pathogenesis and development of endometrial carcinoma.They are favorable prognostic indictors of endometrial carcinoma.
