外鼻恶性肿瘤的手术治疗及Ⅰ期修复效果观察

摘　　要：目的观察在手术根治外鼻恶性肿瘤的基础上Ⅰ期修复创面的效果。方法36例外鼻恶性肿瘤患者在手术彻底根治肿瘤的前提下,根据肿瘤性质、局部缺损情况和患者的年龄分别采取(1)直接缝合或邻近带蒂皮瓣;(2)取全厚皮片游离植皮或双侧鼻唇沟瓣及其他双侧皮瓣修复,或正中额瓣修复;(3)切除受累软骨,用包含有软骨的耳后游离皮瓣修复;(4)全鼻切除者,取游离前臂桡侧皮瓣或额瓣重造全鼻。结果术后1个月患者对外观满意的总例数达33例,不满意3例。结论对外鼻恶性肿瘤行手术治疗,在根治肿瘤的同时行Ⅰ期修复,可提高术后的美学效果和患者的生存质量。Objective To observe the effect of repairing traumatic surface in stageⅠon radical resetting the malignant tumor of external nose.Methods We chose three repairing methods according to the quality of tumor,condition of the tissue lack and age of the patients for 36 patients with malignant tumor of the external nose after radical resection:(1) Suture directly or local pedicle flap;(2) Dematoplasty or two sides of nasolabial flap and middle frontal flap;(3) To reset the affected cartilage and repair it with free flap behind ear;(4) To reset the all external nose and repair with Chinese flap or frontal flap for reconstruction of the external nose.Results One month after operation,33 patients were content with fine appearance;3 patients were not.Conclusion To repair traumatic surface in stageⅠafter resetting malignant tumor of the external nose,it can improve the aesthetics effect after operation and the living quality.
