摘 要:目的探讨磁共振弥散加权成像(DWI)在颅内囊性病变鉴别诊断中的应用价值。方法对62例颅内囊性病变患者分别进行传统MRI及DWI扫描,计算病变区和健侧脑组织的表现弥散系数(ADC)值,比较其间的差别。结果脑脓肿、囊性坏死性脑肿瘤、表皮样囊肿和蛛网膜囊肿在T1WI和T2WI上呈相同的信号改变,在DWI上,脑脓肿和表皮样囊肿呈高信号,囊性坏死性脑肿瘤和蛛网膜囊肿呈低信号。脑脓肿的ADC值(5.739×10-4mm2/s)明显低于囊性坏死性脑肿瘤(28.589×10-4mm2/s),表皮样囊肿的ADC值(10.197×10-4mm2/s)明显低于蛛网膜囊肿(30.098×10-4mm2/s)。结论传统MRI通常无法对颅内囊性病变进行有效鉴别,DWI可应用于颅内囊性病变的鉴别诊断。Objective To study the application value of diffusion-weighted imaging (DWI) in distinguishing intracranial cyst-like lesions. Methods Routine MRI and DWI were obtained in 62 patients with intracranial cystic lesions (35 intra-axial: 10 abscesses,25 cystic necrotic brain tumors; 27 extraaxial: 12 epidermoid cysts,15 arachnoid cysts).ADC values of diseased region and uninjured side brain tissue were calculated and compared. Results The same signal showed in brain abscess,cystic necrotic brain tumor,epidermoid cysts and arachnoid cysts on T_1WI and T_2WI. There was a high signal in brain abscess or epidermoid cysts, while was low in cystic necrotic brain tumor or arachnoid cysts on DWI. ADC value of brain abscess ( 5.739 ×10 -4 mm2/s) was lower than that of cystic necrotic brain tumor (28.589×10 -4 mm2/s). ADC value of epidermoid cysts (10.197×10 -4 mm2/s) was lower than that of arachnoid cysts (30.098×10 -4 mm2/s). Conclusion Routine MR scanning can not efficiently distinguish intracranial cyst-like lesions. DWI can distinguish intracranial cyst-like lesions.
