高度近视眼周边视网膜病变发生的频度及激光屈光术后的变化

摘　　要：目的了解高度近视眼视网膜周边病变发生的频度并初步观察准分子激光原位角膜磨镶术(excimer laser in situ keratomileusis,IASIK)对变性视网膜的影响。方法对286人(568眼)屈光度在-7.00D 以上的 LASIK 术前患者进行散瞳眼底三面镜检查,并对其中存在视网膜变性的115眼术后的眼底情况进行12～18个月的随访。结果 568眼被检者中,发现各种网膜变性125眼,占被检眼数的22%,其中格子样变性53眼(9.33%),霜样变性30眼(5.28%),囊样变性8眼(1.41%)。其他类型变性34眼。发现网膜干性裂洞及亚临床视网膜脱离共11眼(1.94%)。格子样变性以颞侧区域出现的频率最高,占其发生率的77.4%,鼻侧占22.6%,两者间差异显著(P<0.01)。125眼眼底变性中,115眼接受了 LASIK 手术,其中28眼行氩激光光凝治疗后1～6个月接受了手术治疗。术后对以上患者进行12～18个月的随访观察,未发现视网膜脱离。结论屈光手术前,对高度近视患者进行眼底三面镜检查可避免或减少术后视网膜脱离的发生。Purpose To realize the incidence of peripheral retinal diseases in high myopia and verify the influences of Lasik on degeneration retina of high myopia.Methods The fundus were performed with three-mirror contact lens in 286 cases(568 eyes)of myopia befor LASIK,115 eyes of degeneration retina were examined after LASIK 12～18 months.Results Retinal degeneration was detected in 125 eyes(22%).Among them,lattice degeneration 53 eyes(9.33%),frosted degeneration 30 eyes(5.28 %),cystic degeneration 8 eyes(1.41%),other types 34 eyes.Retinal break was detected in 11 eyes(1.94%).115 eyes were operation with LASIK and these patients were examined at 12～18 months after LASIK,not any retinal detachment were found.Conclusion Examining fundus of high myopia with three-mirror contact lens befor LASIK can reduce and avoid retinal detachment after LASIK.
