组织速度成像技术评价肝硬化患者心功能

摘　　要：目的应用组织速度成像技术(TVI)测量二、三尖瓣环舒、缩速度,评价肝硬化患者左、右心室功能。方法对33例肝硬化患者(分A、B、C三组)及正常人20例进行TVI检查,分析二尖瓣环四个位点、三尖瓣三叶瓣环处的TVI平均指标:收缩期峰值速度(Sa),舒张早期峰值速度(Ea),舒张晚期峰值速度(Aa),并计算Ea/Aa比值。结果肝硬化B、C组的左、右心室Ea、Aa、Ea/Aa比值与对照组比较差异均有显著性意义;C组右心室Ea,Ea/Aa值与对照组差异较左心室大(P<0.01,P<0.001)。结论肝硬化患者左、右心室舒张功能减低,肝功能受损程度与右心室舒张功能损害呈正性相关。Objective To assess the left and right ventricular functions in patients with hepatic cirrhosis by tissue velocity imaging(TVI) and the mitral and tricuspid annular diastolic and systolic velocities usere measured.Methods 33 cases with hepatic cirrhosis(A、B、C groups) and 20 healthy controls were studied by TVI.Mean indices of TVI were measured at mitral annular four sites and tricuspid annular level,Sa、Ea and Aa were also measured and Ea/Aa ratio was calculated.Results There were significant differences of the left and right ventricular Ea,Aa and Ea/Aa ratio between B,C groups of hepatic cirrhosis and the control groups,while differences between the right ventricular Ea、Ea/Aa ratio in C group and the control group showed more obviously than those of the left ventricle( P <0.01, P <0.001,respectively).Conclusions In patients with hepatic cirrhosis,the left and right ventricular diastolic functions were decreased and the degrees of hepatic dysfunction was significantly correlated with that the of right ventricular diastolic dysfunction.
