Turner综合征患儿下丘脑－垂体轴功能检测的临床意义

摘　　要：应用复合刺激试验，对２６例确诊为Ｔｕｒｎｅｒ综合征患儿的下丘脑－垂体轴多种激素分泌功能状态进行了检测，并根据检测结果分别给予患儿甲状腺片、吡唑甲氢龙、可乐定等药物治疗。结果：全部患儿中有生长激素分泌低下１２例；中枢性和外周性甲状腺功能低下各４例，后者中有２例经证实为桥本甲状腺炎；５例患儿卵泡刺激素和促黄体生成素分泌相呈现正常青春期前状态；治疗随访超过１年以上的１２例中，除４例骨龄＞１３岁、促甲状腺素分泌正常患儿身高增长效果较差外，余８例均获良好效果。提示：对Ｔｕｒｎｅｒ综合征患儿应尽早检测其下丘脑－垂体轴各项功能，以便根据个体情况予以合理的早期干预，改善其身心发育。ypothalamic-pituitary axis functions in 26 casesof Turner's syndrome were assessed with a combinedstimulating test.The results showed that the peak GHlevels in 12 cases were less than 10μg/L.3 patientsshowed an even TSH response,1 with a delayed TSHpeak,and 4 had high basal values and consistent exag-gerated TSH responces to TRH;all patients showed in-creased basal and peak LH and FSH levels but 5,whose LH and FSH secretory patterns were similar tonormal.12 cases have been treated with individualizedprotocols and followed up for more than 12 months.All of them responsed well except for those with hy-pothyroidism or with a BA less than 13.The authorssuggest that multiple functions of hypothalamic-pitu-itary axia in patients with Turner's syndrome be evalu-atedas eadyas possible for proper medical interventionto improve t heir psychosomatic development.
