卵巢癌患者单克隆抗体放射免疫显像对生存率的影响

摘　　要：用相同方法治疗两组卵巢癌患者并追踪观察。一组在治疗过程中曾作单克隆抗体放射免疫显像（显像组），共２２例，另一组未作过放免显像（对照组），共１５例。两组的年龄分布、病理分类、临床分期及手术治疗、术后化疗等情况均基本相同，仅显像组Ⅲ期患者１４例，对照组Ⅲ期患者８例。单克隆抗体系我室制备的抗卵巢浆液性腺癌抗体，经￣（１３１）Ｉ标记后行腹腔注射。注射后２４～９６小时内行显像检查。追踪观察两组患者的生存情况。结果表明，Ⅲ期患者死亡率，显像组为４２．９％，对照组为７５．０％。根据寿命表法计算，两组的３年生存率分别为，显像组，６３．５％；对照组，２５．０％。经统计学处理尚未见差异有显著性，可能与例数较少有关。本文还介绍了显像组中３例晚期患者的存活情况，并讨论了单克隆抗体放射免疫显像延长患者生存期的机理。wo groups of patients suffered from ovarian carcinoma were studied ret rospecti vely after similartreatments; in one group of 22 patients radioimmunoimaging(RII)had been used, which was not usedin the other group of 15 patients. Other factors were similar in the two group such as hospitalization pe-rid,courses of surgical treatment and regimes of chemotherapy after surgery。The staging according toFIGo was also similar in the two groups except that the number of patients in stage Ⅲ was 14 in the RIIgroup but 8 in the non-RII group.Neither the age distribution nor the histopathological diagnosis was ofmuch difference.￣(131)I labeled monoclonal antibody against ovarian serous adenocarcinoma,generated inour laboratory,was given to all patients intraperitoneally either beofre or after the surgery:7 cases be-fore the initial operation,9 cases before the second lcok operation and 6 cases for follow up after surgeryand chemotherapy. The death rates of stage Ⅲ were 42.9%and 75.0%in the RII group and non-RIIgroup respectively. By means of Kaplain Meir's life table calculation the 3 year survival rates for the twogroups were 63.5%and 25.0%respectively. The RII group was obviously higher than the non-RIIgroup even though not statistically significant.These results were similar to those reporied in the litera-ture.Three very late cases of ovarian carcinoma were reported,in whom RII had been used,are still liv-ing after folfow-up for more than 5 years.The beneficial role of induction of the idiotypic network wasdiscussed, and the possibility of innoculation is under investigation.
