数字减影动脉造影在骨肿瘤的诊断价值

摘　　要：目的：对骨与软组织肿瘤为鉴别其良、恶性质、对周围组织的侵袭情况及是否能进行局部切除，而行ＤＳＡ检查。对象：本组包括恶性肿瘤３例，转移瘤２例，骨巨细胞瘤２例，良性肿瘤１例。处理方法：行股动脉插管（左５例，右３例）做数字减影及术前将供养动脉栓塞。结果：５例恶性肿瘤显示大量增生血管在肿瘤区呈团块状表现，血管迂回不规则，有的可见肿瘤染色。而良性肿瘤则无大量血管增生，可资鉴别。经供养动脉栓塞后２～３日内手术，出血量大为减少。结论：ＤＳＡ检查显示肿瘤部位的血管图像清晰，恶性肿瘤有大量增生血管呈团块状或肿瘤染色，对鉴别良恶性肿瘤有重要意义，并对手术决定及切除提供有益帮助。AbstractObject:Diqital subtraction angiography (DSA ) wasused for the differentiation of hanign and malignant tumors ofbone and soft tissue,and the observation of theirinvasion to the surrounding tissue,in order to make de-cisions for surgical removal.Patients:There were 3 cases of malignant tumors,2cases of metastatic tumors,2 cases of gaint cell tumorof bone and l case of benign tumor.Intervention:The DSA was performed by intra-ar-terial catheterization through the left and right femoralartery respectively in 5 and 3 cases.Results:Tumorous hyperplastic vsssels surroundingandinside the tumor and the tumor mass were seen in 5cases of malignant lesions,“Staining of blood was seenin the tumor”in same cases, No abnorn1ality of the vasculature was found in benign lesions。The nutrientarterv of the tumor was embolized at the end of DSA,the operation was performed 2～3days later and theamount of intraoperative bleeding was markedly re-duced。COnclusiOn:(1) The pictures of maligY1ant tumorsof bOne and soft tissue on DSA were clear and diagnostic,(2 ) The tumorous hyperplastic vessels around andinside the tumor lesions were the characteristic of themalignant tumors while no abnormality of vasculaturewas seen in the benign lesions.(3 ) DSA provided valu-able help for surgical decision,(4 )With the help ofDSA, the amount of inter-operative bleeding can be re-duced.
