摘  要:目的探讨脑出血患者血浆神经元特异性烯醇化酶(NSE)和S-100蛋白含量变化及其临床意义。方法测定32例脑出血患者发病后24小时内、第3天、第7天和第14天血浆内NSE和S-100蛋白含量,并对二者含量的变化及其与脑组织水肿体积、神经功能缺损评分进行相关分析。结果NSE和S-100蛋白在发病后24小时内、第3天、第7天、第14天明显高于正常对照值(P<0.01);第14天时NSE、S-100蛋白含量与脑水肿体积呈明显的正相关(P<0.01)。NSE、S-100蛋白含量与病后24小时内神经功能缺损评分并无明显的相关性(P>0.05);与第3天、第7天和第14天神经功能缺损评分具有正相关性(P<0.01,P<0.05)。结论脑出血患者血浆内NSE、S-100蛋白含量的变化可以反映脑组织损伤和脑组织水肿的情况;血浆内NSE、S-100蛋白含量在一定程度上能够反映神经功能缺损的程度;动态监测NSE、S-100蛋白含量可以作为判断脑组织水肿和评估神经功能缺损程度的临床指标。Objective To explore the change and clinical meaning of neuron-specific enolase(NSE)and S-100 protein in patients with cerebral hemorrhage. Methods The contents of NSE and S-100 protein in blood plasma were measured in the first 24 hours and on the 3rd day, the 7th day, the 14th day in 32 patients with cerebral hemorrhage after onset. The correlation analysis of the contents of NSE and S-100 protein with volume of brain edema and neurological deficit scores were performed. Results The contents of NSE and S-100 protein in blood plasma were siganificantly higher in the first 24 hours and on the 3rd day, the 7th day, the 14th day after onset than those in control group(P<0.01). There was a positive correlation between the contents of the NSE and S-100 protein and volume of brain edema at the 14th day(P<0.01). There was no correlation between the contents of the NSE and S-100 protein and neurological deficit scores(P>0.05),but it was a positive correlation with neurological deficit scores at the 3rd day, the 7th , 14th day(P<0.01,P<0.05).Conclusion The levels of NSE and S-100 protein may reflex the conditions of brain tissue damage and brain edema in patients with cerebral hemorrhage. The contents of NSE and S-100 protein may reflex the degree of neurological deficiency to a certain extent. Detecting the contents of NSE and S-100 protein dynamically may serve as clinical indexes of determining the brain edema and evaluating the degrees of neurological deficiency.
