摘 要:目的检测抑癌基因PTEN编码蛋白、iNOS、COX-2在正常胃粘膜及胃癌组织中的表达情况,探讨PTEN、iNOS及COX-2与胃癌发生发展的关系及作用机理。方法采用免疫组织化学S-P法进行检测。结果10例正常胃粘膜均可见PTEN蛋白的强表达,而40例胃癌组织中28例PTEN蛋白表达明显下降或缺失,差异有显著性(P<0.01),与胃癌的分化程度、浸润深度、TNM分期、淋巴结转移显著相关(P<0.05)。iNOS在胃癌组织中的表达显著高于正常胃粘膜组(72.5%vs10%,P<0.01),与胃癌的浸润深度、淋巴结转移、TNM分期显著相关(P<0.05)。COX-2在胃癌组织中的阳性率为70%,显著高于正常胃粘膜组(P<0.01),与胃癌的淋巴结转移、TNM分期显著相关(P<0.05)。在PTEN蛋白阴性的胃癌组中,iNOS阳性率为93.8%,显著高于PTEN阳性组(P<0.05)。iNOS与COX-2在胃癌组织中的表达呈显著正相关(P<0.01)。结论抑癌基因PTEN编码蛋白在胃癌及胃癌发展过程中表达逐渐下降或缺失,与iNOS之间可能存在相互作用,协同COX-2共同参与了胃癌的发展、转移及浸润。PTEN蛋白表达可作为判定胃癌生物学行为的临床参考指标。Objective To detect the expression of tumor-supressor gene PTEN produced protein、iNOS and COX-2 in gastric carcinoma, and investigate the relationship between these marks and clinical significance in tumorigenesis and progression of gastric carcinoma. Methods The expression of PTEN、iNOS and COX-2 were detected by S-P (Streptavidin-Peroxidase) immunohistochemistry.Results PTEN protein was found strongly positive in ten of normal gastric mucosa,reduced or absent in 28/40 gastric cancers. There were statistically significant correlation between nuclear PTEN expression and the degree of differentiation、the depth of local infiltration、TNM stage、lymph note metastasis(P<0.05).The iNOS expression in gastric carcinoma tissue was higher than in the normal gastric mucosa(72.5% vs 10% P<0.01). There were statistically significant correlation with the depth of local infiltration, lymph note metastasis and TNM stage (P<0.05). Increased expression of COX-2 protein was found in the cytoplasmic region of cancer cells in gastric carcinoma(70%) as compared with that of normal control group(P<0.01). There was a positive correlation between lymph note metastasis or TNM stage and COX-2 protein expression(P<0.05). The positive ratio of iNOS in PTEN-negative gastric cancer was 93.8%,which was much higher than that in PTEN-positive gastric cancer. A close correlation was found between iNOS and COX-2 expression(P<0.01). Conclusion PTEN expression is reduced or absent by advancement of the disease process in gastric carcinoma. There is possible interaction between PTEN and iNOS,iNOS and COX-2 . PTEN expression may be a favorable biologic marker and a useful prognostic indicator in the patients with gastric carcinoma.
