肾盂输尿管癌中P53表达的临床意义  

P53 expression in renal and ureteral cancer

摘　　要：通过免疫组化ＳＰ法，对４６例肾盂输尿管癌石蜡切片进行Ｐ５３基因表达的研究。结果表明：肾盂输尿管癌Ｐ５３表达阳性率为５４．３％（２５／４６），同时发生尿路上皮多器官癌Ｐ５３表达阳性率为８６．７％（７／９），肾盂或输尿管单发癌Ｐ５３表达阳性率为４８．６％（１８／３７）。Ｔ１２１．４％（３／１４），Ｔ２５２．６％（１０／１９），Ｔ３＋Ｔ４９０．８％（１２／１３）。Ｇ１为０，Ｇ２５０％（１５／３０），Ｇ３８３，３％（１０／１２）。随访４６例，Ｐ５３阳性表达术后膀胱癌再发率为３６％（９／２５），Ｐ５３阴性表达术后膀胱癌再发率为３３．３％（７／２１），Ｐ＞０．０５。Ｐ５３阳性表达术后２年内膀胱癌再发率３６％（９／２５），Ｐ５３阴性表达术后２年内膀胱癌再发率为９．５％（２／２１），Ｐ＜０．０５。Ｐ５３阳性表达术后局部复发转移占３２％（８／２５），高于Ｐ５３阴性表达４．８％（１／２１）。Ｐ５３阳性表达５年生存率为４２．９％（６／１４）Ｐ５３阴性表达５年生存率为８１．３％（１３／１５），Ｐ＜０．０５。P53 gene expression was assayed by immunohistochemistry s-p method for paraffin-embeded sections of 46 cases of renal,pelvic and ureteral cancer.The positive rate of P53 expression was 54.3%(25/46) for pelvic-ureteral cancer,being 48.6%(18/37) for cases with synchronous multiorgan urothelial cancer.The positive rate of P53 expression was 21.4 %(3/14) in T1cases,52.6%(10/19) in T2 cases and 90.8%(12/13) in T3-T4. The positive rate was 83.3%(10/12) in G3tumors.50% in G2(15/30) and 0 in G1.The occurrence of bladder cancer within 2 years after surgery was 36%(9/25) in P53 positive cases and only 9.5%(2/21) in P53 negative cases.As to the local reccurrence rate and the metastasis rate, there was no significant difference between the P53 expression positive and the negative cases(32% vs 27.2 %).
