直肠癌生长方式的特征及在预后判定中的意义

摘　　要：本文集中分析研究了直肠癌的肿瘤生长方式的特征，其与诸病理学因素的关系，及不同生长方式对生存率的影响，研究结果表明：呈推进性生长者，由于其多为大的癌巢向周围推挤，周围有较多的纤维组织增生，限制肿瘤向周围扩散，因此它难于向纵横及深层浸润；而且其侵犯小静脉及小淋巴管的能力差；多表现为中、高分化腺癌，切除率及根切率高，患者５、１０年的生存率亦高。而呈浸润性生长者则与此相反。作者认为生长方式在诸生物学特性中起着主导作用，并且对判定预后也有重要的意义。因而有必要深入对生长方式进行研究，并建议将生长方式作为病理诊断中的常规项目进行观察，可能对临床判定预后有指导意义。This study mainly analysed the feature of the growth mode in the rectal carcinoma, especiallythe relation between the feature and the pothologieal elements and the effect of the different growthmode on the survival rates. The results showed: its periphery had much fibroplastic proliferation andlimiting tumor diffusing everywhere because some of the propulsive growth was the big cancer nestpushing towards the periphery.Therefore the propulsive growth less infiltrates longitudinally,transversely and deeply.The capacity of invasing the venula and the small lymphatic vessel was inferior.They mainly present the middle, well-differentiated adenocarcinoma.So its biological feature wasgood and the propability of surviving of patients was high.The auther thought that the growth modeof the tumor was predominant over mang biological feature, and affected others,and had an importance in judging prognosis. It's necessary that the growth mode should be studied further.This studysuggested that the growth mode was observed as the routine examination during the pathologic diangsis, which had much importance in judging the prognosis.
