前交通动脉瘤的分型和手术入路的关系

摘　　要：本文总结了１９９１年以来我科经手术治疗的３６例前交通动脉瘤，根据术中瘤蒂的位置，瘤体的指向，动脉瘤和相关血管的关系，结合脑血管造影所见对前交通动脉瘤进行了分型。用正位像将瘤蒂的位置分为４型，根据双侧Ａ_１段血管的口径和走行曲度，确定主侧血流及瘤蒂和前交通动脉的关系；用侧位像将瘤体的指向划分为３个区域；用斜位像观察双侧Ａ_２和瘤体的关系。上述分型对合理选择手术入路，术中难点预测及意外估计都有较大意义。We sumed up the experience in operation of 36 cases with anterior communicating aneurysm. We classified them according to the position of tumor pedicle, the direction of tumor body, the relation between aneurysm and cerebral vessel and angiography. We divided the tumor pedicle into four types on the orthophoric film.We defined the relation among main blood flow, tumor pedicle and anterior communicating artery according to bilateral A_1 blood vessel ostium,curvature.According to the tumor body direction, we divided them into three fields on the oblique film. We observed the relation between the bilateral A_2 and tumor. We thought that this classification were significant for operation approach option and the difficult prediction during the operation.
