低剂量5－氟尿嘧啶对晚期青光眼小梁切除术后抗瘢痕形成的远期疗效

摘　　要：５４例（６６只眼）原发性青光眼晚期患者行小梁切除术。术后对５４只眼（５－氟尿嘧啶组２９只眼、对照组２５只眼）追踪观察１２～４３个月。结果表明：５－Ｆｕ组术后眼压峰值和波动差值明显低于对照组（Ｐ＜０．０５）；眼压峰值下降率高于对照组（Ｐ＜０．０５）；５－Ｆｕ组不同随访时点手术成功率也明显高于对照组。术后视野变化与眼压下降率及眼灌注压明显相关。ifty-four patients(66 eyes)with the late primary glaucoma were subgrouped randomlyinto 5-Fluorouracil(5-Fu)and control group before trabeculectomy surgery.Thirty-threeeyes in each group were studied.The 5-Fu group received subconjunctival injections of0.5%5-Fu 3mg on the operating table and once a day for 6 days postoperatively.Firty-foureyes(29 eyes in 5-Fu group,25 eyes in control group)were followed up for 12～43 months(30. 83±10. 22 months).The peak of diurnal intraocular pressure (IOP)was 14.38 ±4.02mmHg in 5-Fu group,and 18. 84±3.90mmHg in cntrol group.The percentage of de-creased IOP was 60. 7±14.3%in 5-Fu group,and 47.2±18.1%in control group.The suc-cess rates(IOP≤21mmHg wothout antiglaucoma medicine)at 1,2 and 3 years intervalswers 96.7%,89.41%,89.41%in 5-Fu group, and 96%,57.91%, 33. 84%in controlgroup,There were siginficant linear correlation hetween visual field counts and the percent-age of decreased IOP,perfusion pressure(r=0.273,0.298respectively,P<0. 05).
