晴尔联合低分子量肝素钙治疗急性脑梗死的临床研究

摘　　要：目的观察晴尔(奥扎格雷钠注射液)联合低分子量肝素钙治疗急性脑梗死的疗效与安全性。方法选择80例急性脑梗死患者随机分为2组,晴尔联合低分子量肝素钙加常规治疗组(治疗组)40例,常规药物治疗组(对照组)40例,观察两组患者有效率及血流动力学、血凝分析、D—二聚体、肝肾功能、大便隐血等指标,并观察不良反应。结果治疗组疗效明显优于对照组(μ=3.18,P<0.01)。血液粘度、红细胞压积、红细胞聚集指数明显下降(P<0.05,0.01)。PT,APTT,TT延长(P<0.05,0.02,0.01),FIB降低(P<0.01),D-二聚体77.5%,阳性,无明显不良反应。结论晴尔联合低分子量肝素钙治疗急性脑梗死可明显提高临床疗效,降低血粘度,改善血流动力学,延长PT,APTT,TT,降低FIB,使77.5%D-二聚体阳性,且安全可靠。Objective To observe the efficacy and safety by the union of Sodium Ozagrel injection （SOI） with low molecular weight heparin calcium （LMWH） on patients with acute cerebral infarction. Methods 80 patients with acute cerebral infarction were randomly divided into two groups, each group had 40 patients. Both group were treated with routine medicines, and the treatment group were treated by SOI combined with LMWHC. The effective rate after treatment, hemodynamics, coagulation,D-dimer, hepatic function, renal function and stool occult blood were observed for both groups. And the side-effects were observed. Results The total effective rate was 95 % in the treatment group which was obviously superior to that （75%）in control group （X^2=4.80, P〈0.05）.Whole blood vicosity, hematocrit （HCT）, aggregation of erythrocyte and fibrinogen （FIB） were significantly decreased （P 〈 0.05, 0.0l ） . Prothrombin time （PT）, thrombin time （TT）, activated partial thromboplastin time（APTT） were notable prolonged as compared with those in control group （P〈0.05, 0. 02, 0. 01） . Conclusion The SOI combined with LMWHC could increase the therapeutic effects on acute cerebral infarction which could improve hemodynamics and the positive rate of D-dimer and decrease the blood vicosity, FIB and prolong PT, APTT,TT with safe administration.
