幽门螺杆菌相关性胃炎中核因子-κB与IL-8表达的意义及关系

摘　　要：目的:探讨幽门螺杆菌(Helicobacterpylori,H.pylori)感染时胃黏膜中核因子κB(NFκB)和白细胞介素8(IL8)mRNA及蛋白的表达关系和分布.方法:H.pylori阴性慢性胃炎18例和H.pylori阳性慢性胃炎患者38例,胃黏膜活检标本采用免疫组化法和原位杂交法检测NFκBp65蛋白、IL8mRNA及蛋白的表达.结果:H.pylori阳性慢性胃炎组胃黏膜中NFκBp65蛋白、IL8mRNA和蛋白的表达较H.pylori阴性组显著增强(5.7±3.3vs3.0±2.5,3.7±2.6vs1.1±1.3,4.0±3.0vs1.9±1.6,P<0.01),三者的表达均与胃黏膜慢性炎症程度密切相关(rs分别为0.536,0.474和0.537,P<0.01),与炎症活动性也呈正相关(rs分别为0.575,0.357和0.469,P<0.05),同时与H.pylori密度之间具有显著相关性(rs分别为0.702,0.614和0.713,P<0.01);p65蛋白表达与IL8mRNA和蛋白的表达均呈正相关关系(rs分别为0.666,0.682,P<0.01).结论:H.pylori感染导致胃黏膜NFκBp65和IL8的表达增加,NFκB可能通过调控IL8基因转录,参与了H.pylori相关性胃炎的病理生理过程.AIM： To investigate the relation and distribution of expressions of nuclear factor-κB （NF-κB） p65 protein and interleukin-8 （IL-8） in helicobacter pylori-infected gastric mucosa. METHODS： Gastric mucosal biopsy specimens were obtained respectively from 38 patients with H.pylori-positive gastritis and 18 H. pylori-negative control subjects. The expressions of NF-κB p65 protein, IL-8 mRNA and protein were determined by immunohistochemistry and in situ hybridization. RESULTS： The expressions of p65 protein, IL-8 mRNA and protein were significantly higher in H. pylori-positive gastric mucosa than those in H. pylori-negativecontrols （5.7±3.3 vs3.0±2.5, 3.7±2.6 vs l. l ±1.3,4.0 ~=3.0 vs 1.9 ±1.6, P〈0.01） and they were correlated with the histological severity of activity, inflammation and H. pyloridensity （P〈0.05）. The expression of p65 significantly was correlated with that of IL-8 mRNA and protein（P〈0.01）. There was a positive correlation between IL-8 mRNA and IL-8 protein （ P〈0.01 ）. CONCLUSION： H. pylori infection leads to the overexpression of NF-κB p65 and IL-8 in the gastric mucosa. NF-κB activation may contribute to IL-8 gene transcription and is involved in the pathological process of H. pylori-associated gastritis.
