摘  要:目的:研究HTLV-1病毒通过血脑屏障(BBB)的方式,提示HTLV-1侵犯中枢神经系统(CNS)的机制,从而提示人嗜T淋巴细胞病毒Ⅰ型相关性脊髓病/热带痉挛性截瘫(HAM/TSP)病发病机制和感染途径。方法:通过连接介导多聚酶链反应(LMPCR)分别扩增出5例HAM/TSP患者脑脊液和外周血淋巴细胞中的HTLV-1前病毒DNA片段的序列,然后将扩增产物进行琼脂糖凝胶电泳检测序列的扩增情况,并比较HTLV-1前病毒在脑脊液和外周血淋巴细胞中整合序列的相同机率。结果:HAM/TSP患者脑脊液中存在HTLV-1感染的淋巴细胞数多克隆增殖,而且在脑脊液与外周血中存在HTLV-1病毒整合位点的淋巴细胞克隆。结论:HTLV-1感染可能是通过细胞与细胞间作用而通过血脑屏障的,尤其是通过HTLV-1感染的淋巴细胞迁移至CNS,继而进行增殖并激活CNS中的其它T淋巴细胞,通过对CNS细胞特异性免疫反应和释放细胞因子导致HAM/TSP的病理改变。Objective: To investigate the traffic of HTLV - 1 acrossing the blood - brain barrier and viral entring into the CNS, in order to indicate the mechanism of HAM/ TSP. Methods: CSF mononuclear cells and peripheral blood mononuclear eells(PBMC) were collected from 5 patients with HAM/TSP. The amplification of HTLV - 1 integration sites between CSF and PBMC was compared by Linker - mediated polymerase chain reaction (LMPCR). Results:Such oligoclonal distribution of HTLV - 1 - infected cells was evidenced in the cerebrospinal fluid (CSF) derived from 5 patients with HAM/rSP. Furthermore ,clonal populations of HTLV - 1 - infected lymphocytes sharing the same HTLV - 1 proviral flanking sequences (i. e. integration sites in the cellular DNA) were found in both the CSF and the peripheral blood. Conclusion: HTLV- 1 crosses the blood - brain barrier by migration of HTLV - 1 - infected lymphocytes in vivo. The proliferation of infected cells will lead to pathogenic mechanisms of HAM/TSP.
