摘  要:目的分析研究慢性非特异性下背痛(LBP)患者腰部肌肉活动的sEMG信号特征及其主动运动治疗效应。方法19名正常人对照组和19名非特异性LBP患者分别于主动运动治疗前、后在等惯性力腰部肌肉测试与训练系统上依次完成“等长-动态-等长”运动负荷试验,采集双测L5-S1部位的sEMG信号,计算并比较不同被试者平均肌电值(AEMG)、平均功率频率(MPF)、复杂度[C(n)]和确定性线段百分比(%DET)均值和斜率变化。结果动态运动负荷过程中LBP患者的C(n)下降率明显快于正常人,MPF和C(n)均值明显低于正常人,而%DET均值明显高于正常人;主动运动治疗后,患者MPF和C(n)均值提高,而%DET均值下降。结论运动负荷试验过程中LBP患者腰部肌肉sEMG有多种不同于正常人的信号特征,这些信号特征有望成为LBP诊断和疗效评价的有效指标。Objective To evaluate the diagnostic and active physical therapeutic effects on patients' chronic non-specific low back pain(LBP) based on lumbar muscle sEMG activities during dynamic and isometric exercise by comparing the differences between chronic LBP patients and healthy subjects ,and also between before and after active physical rehabilitation. Method The subjects consisted of 19 male patients with non-specific chronic LBP and 19 male healthy subjects, sEMG signals were recorded bilaterally from L5-$1 during “isometric-dynamic-isometric” lumbar muscle extensions, sEMG signal amplitude ( AEMG ), spectrum ( MPF ), complexity [ C ( n ) ], determinism% ( % DET) average and slope were calculated and compared among healthy subjects, LBP patients before and after active physical rehabilitation. Result C(n) dropped much faster in LBP patients than that in healthy subjects and both averages of MPF and C(n) were lower and %DET was higher than those in healthy subjects during dynamic extension test. After 12 weeks active physical rehabilitation, average C(n) and MPF increased, while average %DET decreased, Conclusion The results demonstrate that sEMG is an effective tool in LBP diagnosis and evaluation of exercise therapeutic effects.
