老年白内障术后视力恢复差异的分析

摘　　要：目的:观察人工晶体植入术后老年患者视功能与术前视力、年龄及术式的关系及其影响因素。方法:纳入2003-03/2004-03在沈阳何氏眼科医院住院确诊为老年性白内障的患者280例342只眼,行白内障超声乳化+人工晶体植入术(261只眼)或行白内障囊外摘除术+人工晶体植入术(81只眼),比较不同年龄、不同术前视力、不同术式的术后视力。结果:按意向处理分析,280例(342只眼)全部进入结果分析。①术前视力水平与术后视力恢复的关系:术前视力>4.0的163只眼术后视力≥4.7的为147只眼,视力提高水平为90.2%(147/163);术前视力≤4.0的167只眼术后视力≥4.7的为87只眼,视力提高水平为52.1%(87/167);差异有显著性意义(χ2=67.87,P<0.001)。②术后视力恢复与年龄的关系:<70岁的白内障患者210只眼,术后视力≥4.7的为175只眼,视力提高水平为83.3%(175/210);≥70岁的白内障患者132只眼,术后视力≥4.7的为59只眼,视力提高水平为44.7%(59/132);差异有显著性意义(χ2=52.02,P<0.001)。③两种术式与术后视力恢复的关系:白内障超声乳化+人工晶体植入术组261只眼术后视力≥4.7的为218只眼,视力提高水平为83.5%(218/261),白内障囊外摘除术+人工晶体植入术组81只眼术后视力≥4.7的为16只眼视力提高水平为19.8%(16/81),差异有显著性意义(χ2=116.37,P<0.001)。结论:术前矫正视力好,手术年龄在50～70岁之间,术中实施超生乳化效果好的,可使术后视功能恢复良好。AIM： To observe the correlation of visual function and vision, age and the type of operation after intraocular implantation and the factors that affect the recovery. pre-operative lens （IOL） METHODS： Totally 280 （342 eyes） aged patients diagnosed as geriatric cataract from March 2003 to March 2004 in Shenyang Heshi Ophthalmology Hospital were involved. The patients were performed cataract phacoemulsification with intraocular lens implantation （PHACO, 261 eyes） or extracapsular cataract extraction with intraocular lens implantation （ECCE, 81 eyes）. The post-operative visual acuity was compared between the cases of different ages and different pre-operative visual acuities and different operation styles. RESULTS： According to intention-to-treat, all the 280 cases （342 eyes） entered the stage of result analysis. ① The correlation between preoperative visual acuity and post-operative visual recovery： there were 163 eyes had visual acuity 〉 4.0 before operation and 147 eyes had visual acuity ≥ 4.7 after operation and the enhanced level of visual acuity was 90.2%（147/163）; there were 167 eyes had the visual acuity ≤ 4.0 before operation and 87 eyes had the visual acuity ≥ 4.7 after operation and the enhanced level of visual acuity was 52.1% （87/167）, significant difference existed （X^2=67.87,P 〈 0.001）.②The correlation between post-operative visual acuity and age ：there were 210 eyes in patients with cataract aged 〈 70 years and 175 eyes of which had post-operative visual acuity ≥ 4.7, the enhanced level of visual acuity was 83.3% （175/210）;There were 132 eyes in patients with cataract aged ≥70 years, and there were 59 eyes with the visual acuity ≥4.7, the enhanced level of visual acuity was 44.7% （59/132）; significant difference existed （X^2=52.02,P 〈 0.001）; ③ The correlation between two types of operations and post-operative visual recovery： Among 261 eyes in the cataract phacoemulsification with intraocular lens implantation group, th
