摘 要:目的:分析人乳头病病毒与肺鳞癌、鳞状上皮化生和支气管黏膜慢性炎及抑癌基因Rb蛋白表达的关系。方法:①实验于2000-07/08在解放军第四军医大学病理学实验室(国家重点实验室)完成。选用解放军第四军医大学西京医院1994-01/1999-12支气管镜检及手术切除标本,其中肺鳞癌、鳞状上皮化生和支气管黏膜慢性炎标本分别为84,41,49例。②采用地高辛标记的人乳头瘤病毒DNA探针,用原位杂交法检测组织中的人乳头瘤病毒DNA存在的情况,并用免疫组化链霉亲和素-生物素-过氧化酶复合物法检测肺鳞癌组织中Rb蛋白的表达。③各组间差异显著性检验采用Fisher确切概率法。结果:①人乳头瘤病毒原位杂交阳性信号紫蓝色颗粒,位于肿瘤细胞核内,Rb免疫组化阳性表达为棕黄色颗粒,Rb表达位于细胞核内。②人乳头瘤病毒检出率:肺鳞癌和鳞状上皮化生组织明显高于黏膜慢性炎组织(49%,20%,4%,P<0.01);肺鳞癌组织明显高于鳞状上皮化生组织(P<0.05)。③84例肺鳞癌中31例检出Rb蛋白表达,其中18例人乳头瘤病毒阳性,13例人乳头瘤病毒阴性,人乳头瘤病毒阳性组与阴性组差异不明显(P>0.05)。结论:①肺鳞癌发病可能与人乳头瘤病毒感染有关,肺鳞癌组织中存在Rb蛋白表达。②人乳头癌病毒杂交阳性组与阴性组Rb蛋白检出率差异显著,说明Rb蛋白积聚与人乳头瘤病毒感染无明显关系。AIM: To analyze the relationship of human papillomavirus (HPV) with squamous cell carcinoma(SqCC) of the lung, squamous metaplasia, chronic inflammation of bronchial mucous membrane and protein expression of tumor suppressor gene Rb. METHODS: ①The experiment was conducted in the laboratory for pathology (national key laboratory) of Fourth Military Medical University from July to August 2004. The specimens following bronchoscopy and operation for resection in Xijing Hospital, Fourth Military Medical University between January 1994 and December 1999 were collected for our study, including 84 cases of SqCC of the lung, 41 of squamous metaplasia and 49 of chronic inflammation of bronchial mucous membrane (controls). ②DNA in HPV tissues was detected by in situ hybridization for HPV using digoxigenin-labeled DNA probe. The protein expression of tumor suppressor gene Rb was measured in SqCC of lung by streptomycinavidin-biotin-complex(SABC) immunohistochemistry. ③Fisher's exact test was used to analyze the significant difference among different tissues. RESULTS: ①Indigo granules were located in tumor nuclei by in situ hybridization for HPV,and the granules was buffy by immunohistochemistry for Rb, Rb mainly in nuclei. ②The detection rate of HPV was significantly higher in the SqCC of the lung and squamous metaplasia tissues than in the chronic inflammation of bronchial mucous membrane tissues(49%,20% vs 4%,P 〈 0.01), and also significantly higher in the first ones than in the second ones above (P 〈 0.05). ③Thirty-one of 84 SqCC tissues were detected with protein expression of Rb, in which 18 was positive HPV and 13 negative HPV, so there was no significant difference between HPV positive and HPV negative groups. CONCLUSION: ①HPV infection may be associated with SqCC of the lung, and there is protein expression of Rb in SqCC tissues.②The detection rate of Rb protein is unremarkably different between positive and negeative HPV by in situ hybridization, indicating t
