乳腺癌中Ⅳ型胶原的表达及临床意义

摘　　要：目的研究Ⅳ型胶原蛋白(ColⅣ)在乳腺癌中的表达及临床意义。方法用免疫组化方法检测108例乳腺癌及8例良性肿瘤组织中ColⅣ的表达情况,分析ColⅣ与乳腺癌患者临床资料间的关系及预后价值。结果8例良性肿瘤ColⅣ表达完整呈闭和线状。108例乳腺癌中,ColⅣ表达完整,局部断裂、碎片状或缺如分别为32例,11例,65例。ColⅣ不完整或缺如与差预后,腋窝淋巴结受累、肿瘤大小、TNM分期呈正相关。结论乳腺癌ColⅣ完整表达或局部断裂者比碎片缺如者生存期长(5年生存期)。因此,Ⅳ型胶原可作为肿瘤侵袭能力的参考指标。Objective To study the clinical significance of expressions of collagen Ⅳ in breast carcinoma. Methods The expressions of ColⅣ in 108 cases of breast carcinoma as well as 8 cases of benign breast tumors were detected with immunohistochemical technique, and analyzed the correlation of the results with clinical prognosis. Results The staining of Col Ⅳ was linear and continuous in 8 cases of benign breast tumors. In 108 cases of breast carcinoma,the continuous, focally disrupted, absent staining were 32.11 and 65 respectively. The disrupted and absent expressions of Col Ⅳ were positive associated with deteriorated prognosis（5 years survival ）, axillary lymph node involvement, masses size and TNM grade, Conclusion The patients with continuous or focally disrupted staining of Col Ⅳ had a longer survival than those with absent expressions . So expressions of Col Ⅳ may be a referenced marker of aggressiveness in breast carcinoma.
