摘  要:目的探讨谷氨酰胺对放射性口腔黏膜炎的治疗作用和对机体功能的影响。方法将48例鼻咽癌放疗的病人随机分成两组:谷氨酰胺组(n=24)和对照组(n=24),谷氨酰胺组使用谷氨酰胺15g/d,用100ml生理盐水稀释,分4次含服,对照组采用100ml生理盐水,使用方法相同,两组病人都采用常规放疗。记录口腔黏膜炎的变化情况和持续的时间,记录体重的变化情况。结果谷氨酰胺组和对照组的发生最大级别口腔黏膜炎和体重下降的差异有显著意义,P值为0.019和0.042;最大级别黏膜炎的平均值、黏膜炎持续时间的平均值和体重下降的平均值的差异也有显著意义,P分别为0.009、0.023和0.001。结论鼻咽癌病人放疗期间含服谷氨酰胺可以减轻放射性口腔黏膜炎的程度和改善病人的机体功能情况。Objective To evaluate the influence of oral glutamine on radiation-induced oral mucositis and patient's performance status in the radiotherapy of nasopharyngeal carcinoma. Methods Fourty-eight patients with nasopharyngeal carcinoma receiving primary irradiation were randomized into glutamine group(15g glutamine in 100ml normal saline) and control group(100ml normal saline), sucked and swallowed test solution four times per day. Patients were treated by 2Gy per fraction daily,5d a week. We evaluated the body weight change, grade change.and duration of oral mucositis. Results The maximum grade of oral mucositis(P= 0. 019) and weight change(P=0. 042)were significantly different between the glutamine group and control group, there were statistical difference in mean maximum grade of oral mucositis (P = 0. 009 ), mean duration of oral mucositis (P = 0. 023) and mean body weight change(P=0. 001) between the two groups. Conclusion Oral glutamine may significantly reduce the severity and duration of oral mucositis and improve the performance status during radiotherapy.
