阿立哌唑与奎硫平治疗精神分裂症疗效和安全性比较

摘　　要：目的比较阿立哌唑与奎硫平治疗精神分裂症的疗效和安全性。方法对符合CCMD-3精神分裂症诊断标准的80例精神分裂症患者,根据随机对照方案分别使用阿立哌唑和奎硫平治疗,疗程共8周。采用阳性症状与阴性症状量表(PANSS)、临床总体印象量表(CGI)进行疗效评定,采用不良反应症状量表(TESS)进行副反应评定。结果治疗8周后,阿立哌唑组显效率为65.0%,有效率为90.0%;奎硫平组显效率为60.0%,有效率为87.5%。两种药物疗效差异无显著性(P>0.05)。阿立哌唑组总不良反应发生率为35.00%,奎硫平组总不良反应发生率37.50%,两组间差异无显著性(P>0.05)。阿立哌唑组和奎硫平组均出现较多的副反应是嗜睡(15.00%:20.00%)、头昏和昏厥(12.5%:15.00%),但两组间差异无显著性(P>0.05)。阿立哌唑组恶心呕吐、头痛的发生率比奎硫平组多(15.00%:2.50%,P<0.05;10.00%:2.50%,P<0.05),奎硫平组口干、食欲减退或厌食比阿立哌唑组多(10.00%:2.50%,P<0.05;10.00%:2.50%,P<0.05)。结论阿立哌唑和奎硫平对精神分裂症疗效相当,不良反应轻,但副反应表现有异同。两种物药均为疗效好、安全性高的的抗精神病药,有利于提高病人用药的依从性。Objective To compare efficacyand safety of aripiprazole and quetiapine in the treatment of schizophrenia. Methods 80 adult patients were recruited in this study. All the cases were diagnosed as schizophrenia in accordance with the CCMD-3 diagnosis standard and randomized into two groups which were treated with aripiprazole or quetiapine for 8 weeks. The Positive and Negative Syndrome Scale （ PANSS ） , Clinical Global Impression（CGI） and Treatment Emergent Side effect Scale（TESS） were used to evaluate efficacy and adverse effects respectively before and at the ends of 2,4,8weeks treatment, Results The significant efficacy rates of aripiprazole was 65.0% and the efficacy rate was 90.0% after 8 weeks while that was 60.0% and 87.5% in quetiapine group . There was no significant difference between the two groups （ P 〉 0, 05 ）. The incidence rate of adverse effect was 35. 00% in aripiprazole group and that was 37, 50% in quetiapine group ,without any significant difference between them（ P 〉 0.05）. The adverse effect either in aripiprazole group or in quetiapine group was drowsiness（ 15.00% ：20.00% ） ,dizziness and faint （12, 5% ：15.00% ）. There was no significant difference between the two groups（ P〉 0.05 ）, The rate of nausea and vomiting , headache in aripiprazole group was higher than that in quetiapine group（ 15.00% ：2, 50% P 〈 0.05 , 10.00% ：2, 50% P 〈 0.05 ） ,. The rate of dry mouth , decrease of appetite or anorexia in quetiapine group was higher than that in aripiprazole group （ 10.00% ：2.50% P〈0.05 , 10, 00% ：2, 50% P〈 0.05）, Conclusion The results suggest that aripiprazole is as effective as quetiapine for the treatment of sihizophrenia and fewer side effects in them ,Although two drugs have different or the same side effects. They both are efficacy and safety antipsychotic drugs in treatment of sihizophrenia . They are beneficial to improve the therapeutic compliance of schizophrenic patients.
