摘 要:目的研究穿透性角膜移植术后角膜厚度最薄点形态特征。方法选择穿透性角膜移植术后3 ̄28个月患者61人,通过ORBSCAN系统获取角膜厚度地形图及最薄点极坐标(T,ρ)。结果61例角膜最薄点平均厚度(T)为(0.336±0.112)mm且平均极距(ρ)为(3.108±0.904)mm;其中最薄点分布,角膜中央3mm区4例(7%)T为(0.439±0.071)mm且ρ为(3.108±0.904)mm;角膜3 ̄5mm区8例(13%)T为(0.328±0.088)mm且ρ为(3.108±0.904)mm;角膜5 ̄7mm区28例(46%)T为(0.320±0.093)mm且ρ为(3.233±0.381)mm;角膜7 ̄9mm区21例(34%)T为(0.340±0.128)mm且ρ为(3.756±0.539)mm。角膜厚度地形周边部呈多峰形、绵延山脉或横断山脉形。结论穿透性角膜移植术后角膜厚度最薄点从中央向周边转移,80%分布在角膜周边5 ̄9mm区域;分布规律表明最薄区位于植片与植床吻合口周围。[Objective] To Research thinnest point characteristics of corneal thickness after penetrating keratoplasty (PKP). [Methods] 61 cases after PKP from 3 to 28 months were examined. Thinnest point polar coordinates (T, ρ) of corneal thickness and corneal thickness topography after PKP were acquired by ORBSCAN system. [Resuits] Of 61 cases, 4 cases (7%) in center 3 mm zone of cornea had mean thickness(T): (0.439±0.071) mm and polar distance(ρ): (3.108±0.904) mm; 8 cases (13%) in 3-5 mm zone had T: (0.328±0.088) mm and ρ: (3.108±0.904) mm; 28 cases (46%) in 5-7 mm zone had T: (0.320±0.093) mm and ρ: (3.233±0.381) mm; 21 cases (34%) in 7-9 mm zone had T: (0.340±0.128) mm and ρ: (3.756±0.539) mm; 61 cases (100%) in 0-9 mm zone had T: (0.336± 0.112) mm and ρ: (3.108±0.904) mm. Corneal thickness topography represents peaks, continuous and transection ridgs. [Conclusions] Corneal thinnest point shifted from center to peripheral cornea and 80% corneal thinnest point scattered over 5-9mm zone of cornea after PKP. Thinnest zone of corneal thickness after PKP scattered over stoma between plant patch and plant bed.
