摘　　要：目的评估无痛性血尿患者尿脱落细胞CK20表达的检测在诊断膀胱癌的前瞻性研究中的价值。方法采用双盲法检测59例无痛性血尿患者尿脱落细胞CK20mRNA的表达,另取20例正常对照标本。分析参数包括肿瘤数目、大小、病理和CK20mRNA标志物。结果17例CK20阳性者中病理活检确诊15例为膀胱癌。该项检测具极高的特异性(90·91%)及阳性预报值(88·24%)。在复发病例中阳性率(58·82%)比初发者中(25·00%)明显要高,P=0·0498。CK20表达与肿瘤分级间无明显相关性。结论CK20阳性表达在血尿患者膀胱癌诊断预测中极具价值。OBJECTIVE： To evaluate the value and potential clinical implications of cytokeratin 20 （CK20） expression in diagnosis of bladder cancer for hematuresis. METHODS：Double blind analysis by RT-PCR for the expression of CK20 mRNA was performed in the urine samples of 59 patients with hematufia. The analytic parameters included tumor size, number of tumor, tumor grade and CK20 expression. RESULTS.. There were 17 cases of CK20 expression with two false-positive results. The specificity （90. 91%） and positive predictive value （88. 24 % ） were much high. The sensitivity was significantly higher in recurrent cases （58. 82% ） than in primary cases （25.00%） ,P = 0. 049 8. No correlation was found between CK20 and tumor grading. CONCLUSION： The expression of CK20 mRNA is a potential marker in diagnosis of bladder cancer for hematuresis.
