膀胱灌注黏膜黏附化疗药物HPC-MMC预防肿瘤复发的疗效观察(附46例报告)

摘　　要：目的探讨黏膜黏附化疗药物HPC-MMC预防肿瘤复发的效果.方法对46例表浅膀胱癌患者于TURBt术后随机分两组行膀胱灌注化疗,黏膜黏附化疗药物组20例,以20 mg/20 ml 1%HPC-MMC膀胱灌注;MMC组26例,以20 mg/20 ml MMC膀胱灌注.疗程1年.HPC-MMC组总灌注次数少于MMC组4次.随访6～36个月.结果 HPC-MMC组未发现出血性膀胱炎,无明显尿路刺激症状.平均随访27.4个月,7例复发,1年复发率为16.7%,3年总复发率38.9%.MMC组平均随访26.8个月,9例复发,1年复发率为22.7%,3年总复发率40.9%.结论黏膜黏附化疗药物HPC-MMC膀胱灌注防止肿瘤复发优于单用MMC,同时可降低副作用,减轻患者痛苦及经济负担.Objective To evaluate the effect of mucousmembrane-adhesive anticancer agent hydroxypropylcellulose （HPC） mitomycin-C （MMC） on preventing recurrence of superficial bladder cancer, Methods Forty-six patients with superficial bladder cancers were randomly divided into two groups after TURBt, Twenty cases in mucousmembrane-adhesive anticancer agent group were instilled intravesically with 20 mg/20 ml MMC mixed with 1% HPC, and 26 cases in the other group were instilled with 20 mg/20 ml MMC alone. During the treatment of one year, the total instillation frequencies in HPC-MMC group were four times less than in MMC group. The follow-up period ranged from 6 to 36 months. Results No hemorrhagic cystitis and bladder irritation were observed in HPC-MMC group. At an average follow-up of 27.4 months in HPC-MMC group, recurrence occurred in 7 patients with the one- and three-year recurrent rate being 16.7 % and 38.9% respectively. In MMC group during an average follow-up of 26.8 months, recurrence occurred in 9 cases with one- and three-year recurrent rate being 22.7 % and 40.9 % respectively.Conclusion The efficiency on preventing recurrence of superficial bladder cancer by intravesical instillation with mucousmembrane-adhesive anticancer agent HPCMMC is better than with MMC alone. As the reduction of drug doses and instillation times, fewer side effects, less pain and cost will be obtained.
