摘 要:目的分析准分子激光原位角膜磨镶术(LASIK)后非接触式眼压计(NCT)测量值的改变及其相关因素。方法对术前屈光度为-1.50^-13.00D的患者103人204眼行LASIK治疗,术后随访1年。术前和术后1、3、6、12个月用NCT测量眼压,比较术前和术后NCT的改变及其相关因素。结果术前NCT平均值15.92±2.55mmHg;术后1年8.74±2.29mmHg,平均减少7.18±2.31mmHg,差异有显著性。多元回归分析显示LASIK后眼压值的改变和术前眼压、等效球镜屈光度、术后角膜曲率有关。结论LASIK后NCT测量读数下降,角膜形态的改变使角膜的生物力学特性发生改变是眼压值降低的真实原因。眼压测量值的下降与术前眼压、等效球镜屈光度、术后角膜曲率有相关性,必须对测量值进行校正后才能更准确地评估术后患者的眼压状况。Objective To evaluate changes in intraocular pressure (IOP) measured by noncontact tonometer (NCT) after laser in situ keratomileusis (LASIK). Method LASIK was performed on 204 eyes of 103 patients for correction of myopia and myopic astigmatism, IOP was measured by noncontact tonometer before and at 1, 3, 6, 12 months after LASIK. The amount of change in IOP was computed, and its relation to different variables was evaluated by regression analysis. Result IOP measured preoperatively was a mean of 15.92 ± 2.55mmHg. IOP measured 1 year postoperatively was a mean of 8.74 ± 2.29mmHg. The value of IOP was reduced with a mean of 7.18 ± 2.31mmHg after LASIK. Multiple regression analysis showed that the decrease in IOP was related to the preoperative IOP, the sphereical equivalent and the postoperative curvature. Conclusion LASIK for myopia produces underestimation of IOP measured by NCT. The reason of the changes in IOP is the ahernation of corneal biomechanical characteristic after LASIK. The decrease in IOP is related to the preoperative IOP, the sphereical equivalent and the postoperative curvature. To evaluate the actual IOP of the postoperative patients correctly, we should take the factors discussed above into account and modify the value.
