岩尖斜坡区肿瘤手术入路探讨

摘　　要：报告了自１９８３年以来收治的２０例岩尖斜坡区肿瘤，其中脑膜瘤９例，神经鞘瘤６例，胆脂瘤５例。肿瘤位于幕下９例，向幕上生长侵及颅中窝１１例，侵及海绵窦３例。手术入路：枕下开颅９例，颞下经小脑幕手术９例，颞枕枕下联合开颅１例，另有１例颞下入路手术后３周行二期枕下开颅手术。手术结果：全切除９例，次全切除１０例，大部切除１例。并着重就岩尖斜坡区肿瘤的手术入路等问题进行扼要的讨论。Since 1983,20 consecutive cases of petroclival tumors were treated surgically.Of them,9were meningiomas,6 were neurinomas and 5 were cholesteatomas.The tumors of 9 patients werelocated in the subtentorium, those of 11 patients extended to middle cranial fossa and the tumorsof 3 patients invaded the cavernous sinus.The surgical approaches used were:suboccipitalcraniectomy in 9 cases;subtemporal-transtentorial approach in 9 cases;and combinedsubtemporal craniotomy and suboccipital craniectomy in 1 case;Two stage operations were donein l patient by ways of subtemporal craniotomy first and suboccipital craniectomy 3 weeks afterfirst operation。Total removal was achieved in 9 cases,subtotal removal in 10 cases,and partialremoval in 1 case,In this article, the operative results and surgical approaches were discusseJd.
