口腔颌面部肿瘤患者T淋巴细胞亚群及细胞因子检测的临床意义

摘　　要：本文对４０例口腔颌面部恶性肿瘤及２０例良性肿瘤患者同时进行外周血Ｔ淋巴细胞亚群、ｓＩＬ－２Ｒ及ＴＮＦ三项指标检测，结果表明，１．良性、恶性患者均有ＣＤ３值明显下降（Ｐ＜０．０５、ｐ＜０．０ｌ），ｃＤ４／ＣＤ８比值明显下降（ｐ＜０．００ｌ），ＣＤ８值明显上升（Ｐ＜０．００１），并且良性高于恶性患者（ｐ＜０．０５），ｃＤ４值恶性患者有下降趋势（ｐ＞０．０５）。２．恶性患者ｓＩＬ－２Ｒ水平明显高于正常人（ｐ＜０．０１），也高于良性患者（ｐ＜０。０５）。３．ＴＮＦ水平三组尚无明显差异（Ｐ＞０。０５）。４．三项检验指标与恶性肿瘤临床分期有一定相关趋势。提示：Ｔ淋巴细胞亚群、ｓＩＬ－２Ｒ检测对口腔颌面部恶性肿瘤诊断及预后判断具有一定参考价值，ＴＮＦ检测的临床意义尚须进一步研究。etiphctal blood levels of T sub-lymphocyte. soluble intetleukin-2 receptor( sIL-2R) andtumor nectosis factor were assessed in 40 patients with oral maxillofacial malignant tumor.20 patients with oral maxillofacial benign tumor. The results present as follows: l) In bothgroups of benign and malignant tumor, Value of CD3 declines significantly<P<0. 05. P< 0. 01respectively), ratio of CD4/CD8 do alike (P <0.001); while value of CD8 rise significantly(P<0.0011: morecover, which is higher in the benigns than in the malignants (P<0.05). 2)Serum levels of slL-2R are much higher in the malignants than in normal controls and thebenigns(P<0.01. P<0.05 respectively) . 3) No difference for scrum levels of TNF among thebenigns the malignants as well as the normal controls. 4) The three assessments areconsidered a correlation with clinical stages of malignant tumor. Our finding indicatethat T sub-IWphocyte and sIL-2R analyses are valuable for diagnosis and prognosis of oralmaxillofacial malignant tumor, clinical value of TNF needs further study.
