支气管肺动脉成形术治疗肺癌的探讨

摘　　要：作者采用支气管肺动脉成形肺切除术治疗中心型肺癌５５例。术式包括隆突切除重建４例，支气管袖状或楔形切除成形４６例，支气管肺动脉联合袖状切除成形５例。术后３年、５年生存率分别为５６．０％（１４／２５）、３７．５％（６６／１６）。作者就手术指征、术式选择、麻醉及手术要点等问题进行讨论。This paper reports the experience in 55 cases of central type lung carcinoma treated bypnenmonectomy with broncho-pulmonary arterioplasty. The models of operation were as follows:pneumonectomywith resection and reconstruction of the carina in 4 cases,lobectomy with sleeve or wedge resection of the bronchus in46 cases and lobectomy with sleeve exciSion of the pulmonary artery and the bronchus in 5 cases. Follow-up showedthat the 3-and 5-year survival rates were 56.0%(14/25)and 37.5(6/l6) respectively。 The operative indication,selection of operative model ,main points of anesthetic and surgical management are discussed。
