大剂量甲氨喋呤静滴后解救剂甲酰四氢叶酸钙的个体化用药

摘　　要：以甲酰四氢叶酸钙（ＣＦ）作为解救剂可使甲氨喋呤（ＭＴＸ）的剂量增至ｌｇ／ｍ２～１０ｇ／ｍ２，但以大剂量甲氨喋呤化疗一定要在合理的血药浓度监测下进行。我们对２４例（３Ｏ人次）恶性肿瘤患者给予大剂量甲氨喋呤为主的化疗，对甲氨喋呤的血药浓度以荧光偏正免疫测定法进行监测，１５例以６小时／０小时血药浓度比值决定甲酰四氢叶酸钙的剂量，１５例采用大剂量ＣＦ为解救剂量。结果显示：两组化疗后的毒性发生率没有明显差异，在一定范围内甲氨喋呤的体内代谢不受甲酰四氢叶酸钙的影响。合理应用甲酰四氢叶酸钙，既能充分发挥甲氨喋呤的抗癌作用，又可以保护正常细胞。Using calclum foliate as rescuer,the dose of methotrexate(MTX)in chemotherpy can be increased to 1-10g/m2 But the use of high-dose MTX in chemotherapy must be kept in a reasonble concentration in serum. High-dose MTXChemotherapy was administered to 30 patients with malignant carcinoma. The serum MTX concentration was moitored byfluorescence polarization immunoassay(FPIA).The dosage of rescuer calcium folinate(CF)was chosen according to the ratioof C6/C0 meethotrexate concentration in 15 patients as one group.In the other group CF of conventional dosage was used in 15patients. The results showed that there was no difference in the post-chemotherapy toxicity incidence between two groups.The metabolism of MTX was not affected by CF in a certain limit.The rational use of CF can both enhance anti-cancer actionof methotrexate and protect normal cells.
