血流速度测定在乳腺肿瘤诊断中的应用价值

摘　　要：作者应用彩色多普勒超声测定了７６例乳腺肿瘤（良性３１例，恶性４５例）内血流收缩期峰速（Ｖｐ）和阻力指数（ＲＩ），并对其临床应用价值作了分析。结果表明恶性肿瘤Ｖｐ值明显高于良性肿瘤，良恶性肿瘤ＲＩ值无显著性差异。统计学分析显示良恶性肿瘤之间的临界流速值为１２ｃｍ／ｓｅｃ，该值对直径２ｃｍ以下乳癌诊断敏感性、特异性和准确性分别为９５．５％、１００％和９６．６％。作者认为流速测定有助于乳腺肿瘤，特别是直径２ｃｍ以下肿瘤的鉴别诊断。The systolic peak flow velocity(Vp)and the resistance index (RI)of 76 cases of breast tumors(31 benign,45 malignant)were measured with color Doppler flow imaging,and their clinical values were evaluated.The resuults showed that the Vp of the malignancy was significantly higher than that of the benign.There was nosignificant differance statistically between the RI of the benign and that of the malignancy,The statistic analysisshowed that a Vp of 12 cm/sec was an optimal threshold value between the benign and the malignancy.With thisstandard,the diagnostic sensitivity,specificity and accuracy for the breast carcinoma smaller than 2 cm in diameterwere 95.5%,100% and 96.6%,respectively,We conclude that the flow velocity measurement is useful for differ-ential diagnosis of the breast tumors,especiaIIy of those smaller than 2 cm in diameter.
