ELISA对幽门螺杆菌感染血清IgG、IgA、IgM抗体检测的诊断价值  

Diagnostic Value of Detecting anti-Helicobacter pylori IgG、 IgM、 IgA and Type Antibodies by Enzyme-liked Immunosorbent Assay

摘　　要：应用酶联免疫吸附试验(ELISA)对无选择性作内镜检查的90例患者血清,分别作抗HPlgG、lgA、lgM特异性抗体的检测,同时与胃粘膜活检组织块的镜检、培养、快速尿素酶试验的结果进行比较。ELISA检测结果显示血清lgG、lgA、lgM抗体的阳性率分别为71.1％(64/90)、56.6％(51/90)、56.6％(51/90),lgG检出率均高于镜检法65.6％(59/90),尿素酶试验55.6％(50/90)、培养法36.7％(33/90)、但lgA、lgM检出率略低于镜检法、而与尿素酶试验相近,高于培养法。三类抗体的灵敏度、特异度分别为81.4％、54.8％(lgG);57.5％、45.2％(lgA);50.8％、32.3％(lgM),提示对特异性lgG抗体水平的检测具有较高的诊断价值。IgG, IgA and IgM specific to Helicobacter pylori (HP) in the sera of 90 patients from whom gastric biopsy specimens were obtained have been detected by ELISA, compared with the results of smear with Gram' s stain, culture and rapid urease test (RUT). The serological assays showed that the positive detection rats of the antibodies IgG, IgA and IgM to HP were 71. 1% (64/90) , 56. 6% (51/90) and 56. 6% (51/90) respectively The positive rate of IgG was the highest among smear 65.6% (59/90), RUT55. 6% (50/90) and culture 36. 7% (33/90), but the positive rates of IgA and IgM were lower than those of smear, and similar to those of RUT, and higher than those of culutre. Sensitivity and Specificity of the above three antibodies were 81. 4% , 54. 8% (IgG) ; 57. 5% , 45. 2% (IgA) ; 50. 8% , 32. 3% (IgM). This study showed that detection for IgG specific antibody was higher capable of diagnostic value.
