输尿管镜下气压弹道碎石术85例临床分析

摘　　要：[背景]探讨输尿管镜下气压弹道碎石术的临床疗效.[病例报告]回顾性分析行输尿管镜下气压弹道碎石术的85例输尿管结石患者临床资料,78例成功,7例失败,其中4例中转开放手术,2例因结石上移至肾盂内而改行体外震波碎石治疗,1例放弃治疗.1例术后发生感染性休克,给予抗感染支持治疗后痊愈;1例术后发现患肾破裂,经保守治疗后痊愈.[讨论]输尿管镜下气压弹道碎石术治疗输尿管结石安全有效,尤其适用于输尿管中下段结石,但也可能发生严重并发症,术前认真选择病例,提高手术技巧对有效降低并发症具有重要意义.BACKGROUND To evaluate the curative effects of pneumatic lithotripsy under ureteroscopy.CASE REPORTS The clinical data of 85 cases of pneumatic lithotripsy by ureteroscopy were retrospectively analyzed.Out of 85 cases,78 cases were succeeded,7 cases were failed in which 4 cases were transfered to open surgery,2 cases with shift of the calculi to the renal pelvis were diverted to extracorporal shock wave lithotripsy,and 1 case was discontinued the procedures.1 case developed infectious shock and another case developed nephritic rupture but recoved with treatment.DISCUSSIONThe pneumatic lithotripsy with ureteroscopy is a safe and effective for treating the ureteral calculus,especially for the calculi located in the middle and lower ureteral segments.But may cause serious complications,therefore think better of patients before operating and advanced surgical skills are important to reduce the occurance of complications.
