摘  要:目的:探讨肿瘤转移相关基因(metastasis-associated gene 1,MTA1)蛋白阳性表达与Ⅰ期非小细胞肺癌(non-small celllung cancer,NSCLC)复发/转移及预后的相关性。方法:应用免疫组织化学法检测60例Ⅰ期NSCLC患者癌组织及30例癌旁正常肺组织中MTA1蛋白的表达情况,应用统计学方法分析MTA1蛋白表达与临床病理特征、复发/转移及预后的相关性。结果:MTA1蛋白在Ⅰ期NSCLC癌组织中阳性表达率显著高于癌旁正常肺组织(P<0.005);MTA1蛋白阳性表达率与肿瘤分化程度(P<0.005)和肿瘤直径(P<0.05)具有相关性,但与患者的年龄、性别、肿瘤组织学类型及吸烟情况无关;MTA1蛋白阳性表达者与阴性表达者在术后5年内发生复发/转移的分别为16例(72.7%)与7例(18.4%),经比较差异有统计学意义(P<0.005);MTA1蛋白阳性表达者的5年生存率为40.9%,显著低于阴性表达者的84.1%(P=0.000 1);COX回归多因素分析结果显示,MTA1蛋白阳性表达是Ⅰ期NSCLC独立的预后因素(HR=5.23,P=0.007)。结论:MTA1蛋白的检测对判断Ⅰ期NSCLC患者复发/转移有一定的预测价值,可以作为其独立的预后指标。Objective:To investigate the relationship between metastasis-associated gene 1 (MTA1) protein expression and recurrence/metastasis and prognosis of stage Ⅰ non-small cell lung cancer (NSCLC).Methods:The expression of MTA1 protein was detected in 60 specimens of stage Ⅰ NSCLC and 30 cases of para-carcinous normal lung tissues using immunohistochemical streptavidin peroxidase (SP) methods. The relationship between MTA1 protein expression and the clinicopathological features, recurrence/metastasis, and prognosis of stage Ⅰ NSCLC was analyzed using statistical methods.Results:The positive expression of MTA1 protein in stage Ⅰ NSCLC was significantly higher than that in the normal lung tissues (P<0.005). The positive expression of MTA1 protein was significantly associated with the degree of tumor differentiation and tumor diameter (P<0.05), while it was unrelated with the age, gender, pathologic type and smoking status. Postoperative follow-up showed that the recurrence/metastasis rate in stage Ⅰ NSCLC patients with positive MTA1 protein expression was significantly higher than those with negative MTA1 expression (72.7% vs 18.4%, P<0.005). The 5-year survival rate in MTA1-positive patients was significantly lower than that in MTA1-negative patients (40.9% vs 84.1%, P=0.0001). COX multivariate regression analysis found that MTA1 protein expression was an independent prognostic factor for stage Ⅰ NSCLC (HR=5.23, P=0.007). Conclusion:The positive expression of MTA1 protein in stage Ⅰ NSCLC tissues was significantly higher than that in the normal lung tissues. Detection of MTA1 protein expression may be helpful to predict both the prognosis and metastasis of patients with stage Ⅰ NSCLC and can be used as an independent prognostic factor.
