免疫组化Envision法CD34染色在不同病变胃粘膜、组织中价值研究

摘　　要：目的探讨用免疫组化Envision法显示CD34在不同病变胃粘膜组织中的价值研究。方法 取临床病理标本,分慢性胃炎56例、慢性萎缩性胃炎50例、肠化生59例、不典型增生61例;无淋巴结转移胃癌组织55例,有淋巴结转移胃癌组织57例。用免疫组织化学Envision法显示各组CD34表达情况,用高倍显微镜观察微血管形态结构并测量微血管密度。结果显微镜下CD34染色阳性呈棕黄色,背景清晰;常表现为分支状、窦隙状、芽孢状、椭圆形、圆形及不规则形等。不同病变胃粘膜组织CD34表现不一,按微血管密度计算:慢性胃炎组为13.17±10.12;慢性萎缩性胃炎组为11.32±7.20;肠化生组为13.19±8.77;不典型增生组为16.58±10.49;无淋巴转移胃癌组为26.72±10.84;有淋巴转移胃癌组为27.69±10.21。微血管密度(MVD)在慢性胃炎组、萎缩性胃炎组、肠化生组组间无差异(>0.05);萎缩性胃炎组、肠化生组、不典型增生组组间无差异(>0.05);慢性胃炎组与不典型增生组有差异(<0.05);慢性胃炎、萎缩性胃炎、肠化生、不典型增生各组与无淋巴转移胃癌组织、有淋巴转移胃癌组织组组间均有显著差异(<0.05);在胃癌组中,无淋巴转移胃癌组织与有淋巴转移胃癌组织组间无差异(>0.05),其组织学病理程度高中分化组、与低分化组间也无差异(>0.05)。结论免疫组化Envision法显示CD34,方法简便,效果稳定,易分辨,可定量分析,是研究微血管变化的良好方法,对胃癌发生发展过程中微血管定量研究具有重要价值,为胃癌诊治提供了参考依据。Objective To investigate immunohistochemical Envision method showed that CD34 in different gastric mucosa lesions in the Value.Methods The clinical and pathological specimens of 56 cases of chronic gastritis,50 cases of chronic atrophic gastritis,intestinal metaplasia in 59 cases,61 cases of atypical hyperplasia;without lymph node metastasis,55 cases of gastric cancer,gastric cancer with lymph node metastasis in 57 cases.Envision immunohistochemistry method showed CD34 expression in each group,with a high power microscope observed capillary morphology and measured Microvessel density.Results CD34 vascular staining under a microscope showed brown,clear background;usually presents a branch-like,sinus gap-like,spore-like,oval,round and irregular-shaped and so on.CD34 expression in different gastric mucosa lesions varies according to microvessel density calculation: chronic gastritis group,13.17 ±10.12;chronic atrophic gastritis group,11.32 ±7.20;intestinal metaplasia group,13.19 ±8.77;atypical hyperplasia group was 16.58 ±10.49;without lymph node metastasis in gastric cancer group was 26.72 ±10.84;with lymph node metastasis in gastric cancer was 27.69 ±10.21.Microvessel density(MVD) in patients with chronic gastritis,atrophic gastritis,intestinal metaplasia were no differences(P> 0.05);atrophic gastritis,intestinal metaplasia,dysplasia were no differences(P> 0.05);chronic gastritis and dysplasia group differences(P 0.05),high degree of histological differentiation of the pathological group,and the poorly differentiated group also no difference(P> 0.05).Conclusions The immunohistochemical Envision method showed CD34,method is simple,stable and easy to distinguish,quantitative analysis,is the study of microvascular changes in the good way,during the development of gastric cancer Microvessel density quantitative study of great value,providing diagnosis and treatment for gastric cancer off reference.
