胃癌患者外周血T细胞亚群测定与患者预后关系的研究

摘　　要：目的研究胃癌患者外周血T细胞CD4/CD8比值与血清IFN-γ、IL-2浓度及预后的相关性。方法应用流式细胞仪检测胃癌患者或健康自愿者静脉血CD4+、CD8+阳性T细胞比例;采用酶联免疫吸附试验(ELISA)检测血清IFN-γ、IL-2浓度。结果健康自愿者CD4/CD8为1.57±0.37,胃癌患者外周血CD4/CD8为1.43±0.71,两者相比差异无统计学意义(P>0.05);以29.6%为参考值将胃癌患者分为CD8+细胞比例增高组或未增高组,两组CD4/CD8比例分别为1.02±0.21、1.64±0.61,差异有统计学意义(P<0.05);两组患者血清IFN-γ浓度(ng/ml)分别为25.42±8.12、6.27±1.77,血清IL-2浓度(ng/L)分别为213.7±21.3、162.3±35.4,两组间差异均有统计学意义(P<0.05);随访结果显示CD4/CD8降低患者具有更高的复发和转移风险(P<0.01)。结论胃癌患者外周血T细胞CD4/CD8降低与血清IL-2、IFN-γ浓度有关,CD4/CD8测定可作为胃癌预后判断的指标。Objective To study CD4/CD8 of T lymphocyte in peripheral blood and its relationship with prognosis of gastric cancer.Methods CD4+、CD8+ T cells in peripheral blood of patients with gastric and health donors was detected by FCM.The concentration of IL-2,IFN-γ in serum was assayed by enzyme linked immunosorbent assay(ELISA).Results The value of CD4/CD8 was 1.57±0.37,1.43±0.71 in health donors or patients with gastric cancer,and the difference was not significant(P>0.05).Accoding to the percentage of CD8+ T cells,patients were divided into two groups: CD4/CD8 decreased(1.02±0.21) or normal(1.64±0.61) group.There was significant difference between two groups(P<0.05).Significant differences were also observed in the concentration of IFN-γ(25.42±8.12 ng/ml vs.6.27±1.77 ng/ml) or IL-2(213.7±21.3 ng/L vs.162.3±35.4 ng/L).Compared to CD4/CD8 normal group,CD4/CD8 decreased group had higher risk of local recurrence and distant metastasis,the difference was statistically significant(P<0.05).Conclusion The decreased value of CD4/CD8 correlated with serum concentration of IL-2 or IFN-γ.The ratio of CD4/ CD8 can be used as one of objective prognosis of patients with gastric cancer.
