摘　　要：目的:评价表面麻醉下白内障超声乳化摘除术联合低度数或负度数人工晶状体植入治疗高度近视眼白内障的临床疗效。方法:高度近视白内障患者(球镜度数-8.00～-20.00D)387例412眼,其中眼轴27～30mm者224眼,眼轴>30mm者118眼。行表面麻醉下低度数或负度数人工晶状体植入术,观察眼轴长度,术前、术后远视力、最佳矫正视力,术后并发症发生情况。随诊时间6～18(平均8.3)mo。结果:眼轴27～30mm术后视力≥0.5者193眼(86.2%),后囊破裂13眼(6.7%),悬韧带断裂2眼(1.0%)。眼轴>30mm组视力≥0.5者58眼占(49.2%),后囊破裂7眼(5.9%),悬韧带断裂2眼(1.7%),视网膜脱离5眼(4.2%)。结论:表面麻醉下低度数或负度数人工晶状体植入术治疗高度近视眼白内障具有术后视力恢复快,屈光状态稳定等优点。但眼轴较长者(>30mm),视力恢复较差。AIM： To evaluate the clinical effects of phacoemulsification combined with topic anesthesia and low/negative power intraocular lens implantation in treating high myopia patients with cataract. METHODS： Three hundred and eighty-seven cases （412 eyes, 224 eyes in 27-30mm axial length group, 118 eyes in 〉30mm axial length group） with high myopia （spherical lens： -8.00- -20.00D） were involved. All eyes had undergone phacoemulsification with topic anesthesia and low/negative power intraocular lens implantation. The ocular axial length, distance visual acuity and the best corrected visual acuity were examined before and after operation. The postoperative complications were also observed. These patients were followed up for 6-18mo, means 8.3mo.RESULTS： The best corrected visual acuity was better than 0.5 in 193 eye （86.2%） of the 27-30mm axial length group and in 58 eyes （49.2%） of the 〉30mm axial length group. The common complications were posterior capsule ruptured and suspensory ligaments broken. In the latter group, 5 eyes （4.2%） had retinal detachment. CONCLUSION： Phacoemulsification with topic anesthesia and low/negative power intraocular lens implantation offers advantage of rapid and better rehabilitation of visual acuity and more stabilization of postoperative astigmatism in treating high myopia patients with cataract, but the recovery of eyes with axial lengths over 30mm was less satisfactory.
