摘 要:目的:研究复发性病毒性角膜炎患者外周血T淋巴细胞亚群、T细胞激活状态及Th1/Th2模式,进一步探讨其免疫发病机制。方法:运用流式细胞仪检测30例复发性病毒性角膜炎患者血浆TH1(IFN-r,TNF-a,IL-2),TH2(IL-10,IL-5,IL-4)细胞因子水平以及外周血淋巴细胞CD3+,CD4+,CD8+T细胞和CD3+HLA-DR+,CD4+HLA-DR+,CD8+HLA-DR+T细胞百分率,并与健康对照组比较。结果:复发性病毒性角膜炎患者血浆IFN-r,IL-2水平有明显上升(P<0.05);IL-4水平下降(P<0.05)。CD4+细胞下降(P<0.05);CD4+HLA-DR+T及CD8+HLA-DR+T细胞上升(P<0.05)。结论:复发性病毒性角膜炎患者T细胞过度活化和Th1/Th2平衡的偏离;Th1激活增强,Th2激活减弱与病毒性角膜炎的复发、迁延有关。AIM: To study the subgroups of activated T lymphocytes and T-cells in the peripheral blood of patients with recurrent viral keratitis and to explore its immune pathological mechanism. METHODS: Flow cytometry was used to detect cytokines of plasma TH1 (IFN-γ TNF-α,IL-2)and TH2 (IL-10,IL-5,IL-4).The percentages of peripheral blood lymphocytes CD3^+, CD4^+, CD8^+ T cells and CD3^+HLA-DR^+, CD4^+HLA-DR^+ and CD8^+HLA-DR^+T cells were also detected with flow cytometry. The results were compared with those of the healthy controls. RESULTS: Compared with the healthy controls, patients with recurrent viral keratitis had the following manifestations: the plasma IFN-γ and IL-2 levels increasedobviously(P〈0.05); IL-4 level decreased (P〈0.05), CD4^+ cell percentage decreased and the differences were significant (P〈0.05),the percentage of CD4^+HLA-DR^+T cellsand CDS^+HLA-DR^+T cells increased (P〈0.05). CONCLUSION: Cellular immune functional disorder,overactivation of T cells, and the imbalance between Th1 and Th2 exist in patients with recurrent viral keratitis. Increased Thl activation and decreased Th2 activation andother existing immune functional disorders may be related to the recurrence and persistence of viral keratitis.
