输注脂肪乳剂对伴高甘油三脂血症胰腺炎大鼠胰腺的影响

摘　　要：目的观察静脉输注脂肪乳剂对伴高甘油三脂(TG)血症胰腺炎大鼠胰腺的病理损害程度。方法SD大鼠静脉滴注蛙皮素(cerulein)诱导水肿性胰腺炎,静脉注射辛基苯基聚氧乙烯醚(TritonWR1339)诱导高TG血症。60只大鼠随机分为5组:A组(胰腺炎组);B1组(轻度高TG血症+胰腺炎组);B2组(轻度高TG血症+胰腺炎+脂肪乳剂组);C1组(中度高TG血症+胰腺炎组);C2组(中度高TG血症+胰腺炎+脂肪乳剂组)。定期测定血清TG值,淀粉酶(AMS)值,测定胰腺湿重,腹水评分以及光镜观察胰腺组织病理损害程度。结果B2组血清TG值、AMS值,胰腺湿重测定,腹水评分及胰腺病理组织学评分与B1组比较差异无统计学意义;C2组血清TG值及AMS值均较C1组明显升高(P<0.05),胰腺湿重测定,腹水评分及胰腺病理组织学评分较C1组加重。结论伴高TG血症的胰腺炎大鼠,当其血清TG值达到一定程度≥(3.86±0.36)mmol/L时,静脉输注脂肪乳剂,将导致血清TG水平进一步升高,胰腺炎病理损害加重。Objecitive To observe the influence of hypertnglyceridemic pancreatitis rats with intralipid on pancreas histo- pathologic damage degree. Methods SD rats were infused with cerulein which induced edematous pancreatitis and received the intravenous injection of triton WR 1339 which induced hypertriglyceridemia. Sixty rats were randomly divided into 5 groups： group A （group of pancreatitis） ; group B1 （group of slight hypertriglyceridemia ＋ pancreatits ）; group B2 （group of slight hypertriglyceridemia ＋ pancreatitis ＋ intralipid） ; group C1 （group of middle hypertriglyceridemia ＋ pancreatitis） ; group C2 （group of middle hypertriglyceridemia ＋ pancreatitis ＋ intralipid）. At regular intervals, the values of serum TG, AMS and the wet weight of pancreas were measured. Ascites was scored. Under the high-microscopy, the histopathologic damage degrees of pancreas were observed. Results The values of serum TG and AMS, the pancreatic wet weight, the ascites and the histopathologic degree of pancreas damage had no significant changes between the group B2 and the group B1. The values of serum TG and AMS in the group C1 （P〈0.05）, the pancreatic wet weight, the ascites and the histopathologic degree of pancreas damage were more serious than in the group C1 too. Conclusion When serum TG values of hypertriglyceridemic pancreatitis rate reached a level （≥3.86± 0.36 mmol/L）, intralipid could elevate serum TG level and aggravate the histopathologic damage in pancreatitis.
