羟基磷灰石增高下颌牙槽嵴及其延展术

摘　　要：作者报告了８４例应用羟基磷灰石（ｈｙｄｒｏｘｌａｐａｔｉｔｅ，ＨＡ）增高下颌剩余牙槽嵴及其二期延展术５年随访结果，详细描述了ＨＡ一期粘骨膜下植入术和二期降低口底、前庭沟加深和皮肤移植成形术。通过一期手术增高了牙槽嵴，二期手术则有效地消除了唇、颊系带和肌肉的干扰，增加了义齿的固位和稳定，增加了人工牙槽嵴覆盖组织的磨擦耐受性，减少配戴义齿时的疼痛和不适。８４例５年随访结果表明，这种方法明显优于单纯ＨＡ增高术。ＨＡ是一种理想的骨代用材料，绝大多数病例没有发生ＨＡ吸收。Theresultsof5yearsfolow-upof84casesofmandibularresidualridgeaugmentedwithhydroxlapatite(HA),folowedbyridgeextension,arereported.Thefirstsurgicalprocedures,subperiostealHAinsertion,andthesecondstageoftotalloweringofthefloowofthemouth,vestibuloplastyandskingraftaredetailedlydescribed.Theridgeisaugmentedbythefirstsurgery,disturbanceofthefraenumsofthelip,bucalandtongueandmusclesareeliminatedbythesecondsurgery,toincreasedentureretentionandstability,improvewear-resistingoftissuecoveredovertheartificalridgeandeliminatepainanduncomfortwhenwearingdenture.Theresultsoffiveyearsfolow-upshowthattwostagesreconstructionarebeterthanHAinsertiononly.HAisakindofide-almaterialofbonereplacement,whichisnotresorbedinmostcases.
