摘  要:目的探讨胎儿心脏畸形与其他结构及染色体异常的关系.方法采用彩色多普勒对孕16~40周胎儿行48 186人次的系统胎儿检查及超声心动图检查,对产前超声诊断并经病理或分娩后随访证实的先天性心脏病(先心病)胎儿资料进行回顾性分析.结果确诊88例先心病,其中单纯心脏畸形39例,心脏畸形合并心外畸形49例.产前漏诊14例,其中室间隔缺损12例,法洛四联症2例.88例中70例终止妊娠,18例行脐血染色体检查,其中7例染色体异常,以18-三体多见.结论胎儿先心病以复杂先心病多见,常合并多器官畸形,与染色体异常发生有关.Objective To investigate the relationship between congenital heart defects and abnormalities of other organs and chromosomes in fetus. Methods A total of 48186 person-time of systematic fetal ultrasonography and fetal echocardiography were performed on the fetuses at gestational age of 16-40 weeks. The data of the fetuses who were diagnosed with congenital heart diseases by prenatal ultrasonography and whose conditions were confirmed by postnatal pathology and follow-up were reviewed. Results Congenital heart diseases was diagnosed in 88 cases, among whom 39 cases had simple fetal heart defects and 49 cases had heart defects with other fetal deformities. Fourteen cases were missed for the diagnosis ( 12 cases had ventricular septal defects and 2 cases had tetralogy of Fallot). Of the 88 cases, 70 underwent termination of pregnancy; 18 underwent fetal blood sampling (of them 7 had chromosomal anomalies). Conclusions Color Doppler ultrasound plays an important role in diagnosis of fetal congenital heart defects. The most common types were complex congenital heart diseases which were often complicated with multiple organ abnormalities and chromosomal anomalies.
